摘　　要：异基因造血干细胞移植(allo-HSCT)是目前治疗恶性血液疾病的主要方法,单倍体造血干细胞移植(haplo-HSCT)作为其中一种方法,可使多数患者明显获益,但易并发移植物抗宿主病(GVHD)。目前治疗GVHD的一线方案为糖皮质激素,但仍有部分患者对激素耐药,导致预后不佳。间充质干细胞(MSCs)是一种具有自我更新能力和多向分化能力的非造血干细胞,可调节多种免疫细胞,并在一定条件下发挥免疫抑制作用,对GVHD有良好的治疗作用。本文报道1例haplo-HSCT后并发重度胃肠道GVHD (GI-GVHD)的患者经MCSs治疗后痊愈,并进行文献复习,旨在为临床治疗激素耐药型重度GI-GVHD患者提供思路。Allogeneic hematopoietic stem cell transplantation(allo-HSCT) is the main method for the treatment of hematological malignancies. Haploid hematopoietic stem cell transplantation(haplo-HSCT), as one of the methods,can bring significant benefits to most patients, but it is prone to be complicated by graft-versus-host disease(GVHD). At present, glucocorticoids are the first-line regimen for the treatment of GVHD, but some patients are still resistant to hormones, resulting in poor prognosis. Mesenchymal stem cells(MSCs) are non-hematopoietic stem cells with self-renewal ability and multidirectional differentiation ability, which can regulate a variety of immune cells and exert immunosuppressive effects under certain conditions, and have a good therapeutic effect on GVHD. In this paper, a case of severe gastrointestinal GVHD(GI-GVHD) after haplo-HSCT was reported and recovered after MCSs treatment, and the literature was reviewed, aiming to provide ideas for clinical treatment of severe GI-GVHD patients with hormone resistance.
