不同病情及病程CAP新生儿外周血维生素D、淋巴细胞亚群、中性粒细胞吞噬功能比较及相关性分析

摘　　要：目的比较不同病情及病程社区获得性肺炎(CAP)新生儿外周血维生素D(VD)、淋巴细胞亚群、中性粒细胞(NE)吞噬功能,并分析VD含量和免疫功能的相关性。方法62例CAP新生儿根据病情分为轻症者40例(轻症组)、重症者22例(重症组),根据病程分为急性期(入院后24 h内,急性组)、恢复期(治疗7 d病情好转时,恢复组),另选同期健康体检新生儿30例(对照组),比较各组外周血VD含量[25(OH)D]、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))、B淋巴细胞亚群(CD19^(+))、NK细胞(CD56^(+))及NE吞噬率、吞噬指数。结果与对照组比较,轻症组和重症组外周血25(OH)D、CD3^(+)细胞、CD4^(+)细胞、CD56^(+)细胞、NE吞噬率、NE吞噬指数减少,CD19^(+)细胞增加(P均<0.05);与轻症组比较,重症组外周血25(OH)D、CD3^(+)细胞、CD4^(+)细胞、CD56^(+)细胞、NE吞噬率、NE吞噬指数减少,CD19^(+)细胞增加(P均<0.05)。与对照组比较,急性组和恢复组外周血25(OH)D、CD3^(+)细胞、CD4^(+)细胞、CD56^(+)细胞、NE吞噬率、NE吞噬指数减少,CD19^(+)细胞增加(P均<0.05);与恢复组比较,急性组外周血25(OH)D、CD3^(+)细胞、CD4^(+)细胞、CD56^(+)细胞、NE吞噬率、NE吞噬指数减少,CD19^(+)细胞增加(P均<0.05)。急性组VD含量与CD3^(+)、CD4^(+)、CD56^(+)细胞及NE吞噬率、吞噬指数呈正相关(r分别为0.354、0.321、0.386、0.520、0.559,P均<0.05),与CD19^(+)细胞呈负相关(r为-0.342,P<0.05),与CD8^(+)细胞无相关性(r为0.130,P>0.05)。结论CAP新生儿外周血VD含量减少,细胞免疫水平降低,NE吞噬功能减弱,且与病情轻重和病程进展密切关联,急性期VD含量与淋巴细胞亚群、NE吞噬功能之间具有一定的相关性。Objective To compare the levels of vitamin D(VD),lymphocyte subsets,and phagocytosis of neutro⁃phils(NE)in peripheral blood of neonates with community-acquired pneumonia(CAP)in different conditions and course,and to analyze the correlation between VD content and immune function.Methods Sixty-two neonates with CAP were di⁃vided into the mild group(40 cases)and severe group(22 cases)according to their condition;they were divided into the acute phase(within 24 h after admission,the acute group)and recovery phase(seven days after treatment when the condi⁃tion improved,the recovery group)according to the course of disease.In addition,30 healthy neonates who had physical examination in the same period were selected(the control group),and the VD content[25(OH)D],T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+)),B lymphocyte subsets(CD19^(+)),NK cells(CD56^(+)),NE phagocytosis rate and phagocytosis index were compared among groups.Results Compared with the control group,25(OH)D,CD3^(+)cells,CD4^(+)cells,CD56^(+)cells,NE phagocytosis rate and NE phagocytosis index decreased in peripheral blood of the mild and severe groups,but CD19^(+)cells increased(all P<0.05).Compared with the mild group,25(OH)D,CD3^(+)cells,CD4^(+)cells,CD56^(+)cells,NE phagocytosis rate and NE phagocytosis index decreased in the peripheral blood of the severe group,but CD19^(+)cells increased(all P<0.05).Compared with the control group,25(OH)D,CD3^(+)cells,CD4^(+)cells,CD56^(+)cells,NE phagocytosis rate and NE phagocytosis index decreased in the peripheral blood of the acute group and recovery group,but CD19^(+)cells increased(all P<0.05).Compared with the recovery group,the peripheral blood 25(OH)D,CD3^(+)cells,CD4^(+)cells,CD56^(+)cells,NE phagocytosis rate and NE phagocytosis index decreased in the acute group,but CD19^(+)cells increased(all P<0.05).The content of VD of CAP neonates at the acute phase was positively correlated with CD3^(+),CD4^(+),CD56^(+)cells,NE phagocytosis rate and phagocytosis index(r=0.354,0.321,0.386,0.520,0.559,all P<0.0
