摘　　要：目的:了解注射用红色诺卡氏菌细胞壁骨架(Nr-CWS)的临床应用情况,分析其用药合理性,为临床合理使用该药提供参考。方法:利用某院合理用药智能管理系统回顾性调取2021年所有使用注射用Nr-CWS的住院患者病历,统计患者性别、年龄、就诊科室、用药适应证、药品用法用量、联合用药、实验室检查等信息,进行用药合理性分析。结果:共收集211份住院患者病历,其中存在不合理现象的病历数共69份(32.70%);共发生不合理用药137例次,表现为未监测免疫功能(69例次,50.36%)、用法不适宜(62例次,45.26%)、重复用药(5例次,3.65%)和给药剂量不适宜(1例次,0.73%)。结论:临床中使用Nr-CWS不合理情况较多,主要是未进行与有效性评估相关的实验室检查以及用法不适宜,临床医师、临床药师及医院相关部门应加强对该药的医嘱点评以及药品管理,避免医疗资源浪费,共同促进合理用药。Objective:To understand the clinical application and rationality of Nocardia rubra cell wall skeleton(Nr-CWS for injection in a hospital,and provide references for the rational drug use in clinic.Methods:The medical records of the inpatients who used Nr-CWS for injection in 2021 in a hospital were retrospectively extracted from the intelligent management system for rational drug use and analyzed in terms of the gender,age,department of disease,drug indications,usage and dosage,combined medication,and lab examinations,etc.Results:A total of 211 medical records were collected,of which 69 records(32.70%)were unreasonable with 137 times of irrational drug use including unmonitoring immune function(69 times,50.36%),inappropriate usage(62 times,45.26%),repeated medication(5 times,3.65%)and inappropriate dose(1 case,0.73%).Conclusion:There were several unreasonable phenomenen in the use of Nr-CWS in hospital,mainly reflected in the lack of effectiveness assessment and inappropriate usage.Clinicians,clinical pharmacists and relevant departments of the hospital should strengthen the comment of medical orders and management of Nr-CWS in order to avoid the waste of medical resources,ensure the safety and rational drug use.
