摘 要:目的:探究药品规格的改变对化疗药物浪费的影响程度。方法:通过HIS系统回顾性收集2021年9–11月PIVAS中存在浪费的化疗药品处方信息,根据药品实际使用量和药品规格之差计算其浪费量。对产生浪费的处方中排名前5位的药物进行方案设计,在只提供小规格或者同时提供大小两种规格的情况下,重新分析处方信息,计算其浪费量,并与实际情况下的浪费量进行对比。结果:共有3509张处方存在浪费现象,涉及46种药物,药物总浪费量为314898 mg。排名前5位的药物分别为顺铂、阿糖胞苷、奥沙利铂、亚叶酸钙、依托泊苷。若只使用小规格,则药物浪费量可减少6100~123600 mg;若同时使用大小两种规格,则药物浪费量可减少6320~124142 mg。结论:优化药品规格能大幅度减少药物浪费现象。Objective:To investigate the effect of the change of drug specifications on chemotherapy drugs waste.Methods:Information of the chemotherapy drugs prescriptions with waste in pharmacy intravenous admixture service(PIVAS)from September 2021 to November 2021 were retrospectively collected by the hospital information system(HIS)and the drug waste was calculated based on the differences between actual drug usage and drug specifications.Drugs with the top five waste and information of their prescriptions were re-designed and re-analyzed when only small specifications or both small and large specifications were available.The designed waste was calculated and compared with the real one.Results:There were a total of 3509 prescriptions with waste,46 drugs involved with a total waste of 314898 mg.The top five drugs generating wasted prescriptions were cisplatin,cytarabine,oxaliplatin,calcium folinate,and etoposide.The amount of drug waste was reduced by 6100–123600 mg if only small specifications were available and reduced by 6320–124142 mg if both large and small specifications were available.Conclusion:Optimizing drug specifications could significantly reduce drug waste.
