布洛芬混悬液致急性荨麻疹伴血管神经性水肿1例

摘　　要：1例5岁4个月女性患儿,因急性咽炎发热服用布洛芬混悬液。首次服药2 h后,患儿出现双眼睑及四肢关节轻微浮肿、局部少量皮疹。家长自行为其涂抹青草药膏,1 h后患儿浮肿及皮疹基本消退。当晚患儿因发热再次口服布洛芬混悬液,服药1 h后患儿全身瘙痒并出现风团样皮疹,双眼睑及四肢关节处出现浮肿,下嘴唇发红并肿胀,再次涂抹药膏后症状未见缓解,并逐渐加重,遂入我院急诊科就诊。经临床诊断,考虑为布洛芬混悬液所致急性荨麻疹伴血管神经性水肿,立即给予地塞米松磷酸钠注射液静脉滴注,苯海拉明注射液肌注、氯雷他定片口服治疗。1 h后,患儿全身皮疹及四肢关节处肿胀逐渐消退,双眼睑及下嘴唇肿胀明显减轻。次日,患儿上述症状完全消失。A 5-year-and-4-month-old female child patient took ibuprofen suspension because of acute pharyngitis with fever.2 hours after the first dose,she developed slight swelling of double eyelids and joints of limbs,and a small amount of skin rash,which were relieved 1 hour after being given Qingcao ointment by her parents.On that night,she took ibuprofen suspension again due to fever.1 hour later,she developed pruritus with rubella in the whole body,edema in the eyelids and joints of limbs,and red and swelling in the lower lip,which was not relieved by using Qingcao ointment again and gradually aggravated.And then she was admitted to the emergency department of hospital.Acute urticaria with angioneurotic edema caused by ibuprofen suspension was considered and intravenous drip of dexamethasone sodium phosphate injection,intramuscular injection of diphenhydramine injection,loratadine tablets was immediately given to her.The skin rash and swelling of joints of limbs were gradually relieved after 1 hour and the swelling of both eyelids and lower lips was significantly reduced.The above symptoms disappeared completely the next day.
