摘 要:目的 探析精细化护理干预对老年重症糖尿病足伤口护理质量的改善效果。方法 选择2021年6月—2022年3月莆田学院附属医院收诊的老年重症糖尿病足患者共计84例为研究对象,以就诊先后顺序分为常规组(2021年6—10月)、研究组(2021年11月—2022年3月),每组42例。予常规组对象常规护理措施,予以研究组对象精细化护理措施。对比两组血糖水平、溃疡面积、疼痛(Visual Analogue Scale,VAS)评分、踝肱指数、护理满意度。结果 研究组血糖水平[FBG(5.03±1.77)mmol/L、2 hPG(5.56±1.32)mmol/L、HbA1c(4.77±0.82)%]、溃疡面积(255.08±15.31)mm^(2)、VAS评分(2.19±1.06)分明显低于常规组,差异有统计学意义(P<0.05)。研究组护理满意度97.61%、踝肱指数(1.34±0.20)均高于常规组,差异有统计学意义(P<0.05)。结论精细化护理干预可显著提高老年重症糖尿病足伤口的护理质量,增强足功能、减少溃疡。Objective To investigate the effect of refined nursing intervention on the improvement of wound care quality of geriatric critically ill diabetic foot.Methods 84 cases of elderly patients with severe diabetic foot admitted to the Affiliated Hospital of Putian College from June 2021 to March 2022 were selected for the study. The patients were divided into the conventional group(June to October 2021) and the study group(November 2021-March 2022) according to the order of visit, with 42 cases in each group. The subjects in the conventional group were given conventional nursing measures. Refined nursing measures were given to the subjects in the study group. The blood glucose level, ulcer area, pain(visual analogue scale, VAS) score, ankle-brachial index, and nursing satisfaction were compared between the two groups.Results Blood glucose levels [FBG(5.03±1.77) mmol/L, 2 hPG(5.56±1.32) mmol/L, HbA1c(4.77±0.82) %], ulcer area(255.08±15.31) mm^(2), and VAS score(2.19±1.06) points in the study group were significantly lower than those in the conventional group, and the difference between the two groups was statistically significant(P<0.05). The nursing satisfaction rate 97.61% and the ankle-brachial index(1.34±0.20) of the study group were all higher than those of the conventional group, and the difference between the two groups was statistically significant(P<0.05).Conclusion Refined nursing intervention can significantly improve the quality of care for elderly severe diabetic foot wounds and enhance foot function to reduce ulcers.
