利拉鲁肽对2型糖尿病MCI患者认知功能及尿ACR、PCX水平的影响

摘  要:目的探讨利拉鲁肽对2型糖尿病轻度认知功能损害(mild cognitive impairment,MCI)患者认知功能及尿白蛋白与肌酐比值(ACR)和足细胞标志蛋白(PCX)的影响。方法选取2019年10月—2020年1月韶关市铁路医院收治的65例2型糖尿病MCI患者,按照随机数表法分为对照组(n=32,常规治疗)和观察组(n=33,在对照组基础上给予利拉鲁肽),对比两组血糖水平、ACR与PCX的水平及认知功能评分(MoCA)。结果治疗后,观察组患者空腹血糖(FPG)(6.01±1.51)mmol/L、餐后2 h血糖(2 hPG)(7.44±2.12)mmol/L、糖化血红蛋白(HbA1c)(6.05±1.52)%均明显低于对照组,差异有统计学意义(t=4.978、6.626、2.934,P<0.05);治疗后,观察组患者ACR(4.02±1.51)μg/L,PCX(5.43±2.12)mg/μmol均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组的MoCA评分均高于对照组,差异有统计学意义(P<0.05)。结论2型糖尿病MCI患者使用利拉鲁肽治疗可降低血糖,减少蛋白尿及对肾脏的损伤,提高认知功能。Objective To investigate the effects of liraglutide on cognitive function and urinary albumin-to-creatinine ratio(ACR)foot cell marker protein(PCX)in patients with mild cognitive impairment(MCI)in type 2 diabetes mellitus.Methods 65 patients with type 2 diabetes mellitus MCI admitted to Shaoguan Railway Hospital from October 2019to January 2020 were selected.They were divided into control group(n=32,conventional treatment)and observation group(n=33,liraglutide was given on the basis of the control group)according to the random number table method.The blood glucose levels,levels of ACR and PCX and cognitive function scores(MoCA)were compared between the two groups.Results After treatment,fasting blood glucose(FPG)(6.01±1.51)mmol/L,2 h postprandial blood glucose(2 hPG)(7.44±2.12)mmol/L,and glycosylated hemoglobin(HbA1c)(6.05±1.52)%in the observation group were significantly lower than in the control group,and the differences were statistically significant(t=4.978,6.626,2.934,P<0.05);after treatment,the ACR(4.02±1.51)μg/L and PCX(5.43±2.12)mg/μmol in the observation group were lower than in the control group,and the differences were statistically significant(P<0.05);after treatment,the MoCA scores in the observation group were higher than in the control group,and the differences were statistically significant(P<0.05).Conclusion Treatment with liraglutide in type 2 diabetic MCI patients can lower blood glucose,reduce proteinuria and damage to the kidneys,and improve cognitive function.
