摘　　要：目的:探究心力衰竭伴高血压患者应用磁共振成像评估左心室形态变化的临床价值。方法:选取2019年10月—2021年10月宁夏医科大学总医院收治的82例心力衰竭伴发高血压患者为实验组,另取同期于我院确诊的41例单纯心力衰竭未并发高血压者为对照组,均进行磁共振成像检测,分析两组磁共振相关指标差异。结果:两组患者左心室扩张指标左心室舒张末期容积、左心室收缩末期容积、左心室舒张末期内径、射血分数差异均无统计学意义(P>0.05);实验组患者的左心室质量显著高于对照组(P<0.01);实验组患者左心室各室壁厚度均大于对照组,其中前间隔和后间隔最大室壁厚度差异显著(P<0.01);实验组患者的右心室运动异常占比显著高于对照组(P<0.01),但二尖瓣反流和纤维化占比与对照组差异不显著(P>0.05)。结论:与不伴发高血压的心力衰竭患者相比,并发高血压心力衰竭患者磁共振成像存在一定相似,但伴发高血压心力衰竭患者的左心室壁厚度明显升高,应加强对此类患者的监测。Objective To explore the clinical value of magnetic resonance imaging(MRI)in evaluating the morphological changes of left ventricle in patients with heart failure and hypertension.Methods From October 2019 to October 2021,82 patients with heart failure complicated with hypertension from General Hospital of Ningxia Medical University were selected as the experimental group,and 41 patients with simple heart failure without hypertension were selected as the control group.All of them were examined by MRI,and the differences of MRI related indexes between the two groups were analyzed.Results There was no statistically significant difference in LVEDV,LVSV,LVDD and LVEF between the two groups(P>0.05);The left ventricular mass in the experimental group was significantly higher than that in the control group(P<0.01);The left ventricular wall thickness in the experimental group was larger than that in the control group,and the difference between the maximum wall thickness of anterior septum and posterior septum was significant(P<0.01);The percentage of abnormal right ventricular movement in the experimental group was significantly higher than that in the control group(P<0.01),but the percentage of mitral regurgitation and fibrosis had no significant difference(P>0.05).Conclusion Compared with patients with heart failure without hypertension,the MRI of patients with heart failure complicated with hypertension is similar,but the left ventricular wall thickness of patients with heart failure complicated with hypertension is obviously increased,so the monitoring of such patients should be strengthened.
