摘  要:目的:探讨磁共振成像(MRI)弥散加权成像(DWI)在骨肌肿瘤良恶性鉴别与诊断中的应用价值。方法:选取2017年1月-2021年10月睢宁县人民医院收治的骨肌肿瘤患者42例及体检健康者21例作为研究对象,42例骨肌肿瘤患者根据手术病理检查确诊结果将骨肌肿瘤患者分为良性组与恶性组,各21例,健康者21例为对照组。比较三组在不同b值检查下的ADC值,分析良性组与恶性组MRI检查DWI序列影像特征。结果:b值600 s/mm^(2)、800 s/mm^(2)、1 000 s/mm^(2)时,恶性组ADC值小于对照组,良性组ADC值大于对照组,差异有统计学意义(P<0.05)。恶性组出现周围水肿、液化坏死及静态增强概率高于良性组,差异有统计学意义(P<0.05)。结论:骨肌肿瘤良恶性鉴别与诊断中应用MRI检查DWI序列扫描,可以通过对比分析患者在不同b值下ADC值改变,在MRI影像特征辅助下,对骨肌肿瘤进行准确的良恶性鉴别诊断,应用价值较高。Objective: To explore the application value of magnetic resonance imaging(MRI) diffusion weighted imaging(DWI) in the differentiation and diagnosis of benign and malignant musculoskeletal tumors. Methods: A total of 42 patients with musculoskeletal tumors diagnosed and treated in Suining County People’s Hospital and 21 subjects for physical examination from January 2017 to October 2021 were selected as the study subjects. According to the diagnosis results of surgical pathology examination, 42 patients with musculoskeletal tumors were divided into benign group and malignant group, with 21 cases in each group. The 21 healthy subjects were included as the control group. The ADC values under different b-value examinations of the three groups were compared, and the characteristics of DWI sequence in MRI examination were analyzed between the benign group and the malignant group. Results: When the b values were 600 s/mm^(2), 800 s/mm^(2)and 1 000 s/mm^(2), the ADC values of the malignant group were lower than those of the control group, and the ADC values of the benign group were higher than those of the control group, and the difference was statistically significant(P<0.05). The probability of peripheral edema, liquefaction necrosis and static enhancement in the malignant group was higher than that in the benign group, and the difference was statistically significant(P<0.05). Conclusion: The application of MRI DWI sequence scan in the differentiation and diagnosis of benign and malignant musculoskeletal tumors can make an accurate differential diagnosis of benign and malignant musculoskeletal tumors with the assistance of MRI image characteristics through comparative analysis of ADC value changes in patients with different B values. It has a high application value.
