摘  要:目的探讨DICOM影像分析软件OsiriX在下腰椎椎弓根形态学测量中的应用价值。方法收集2019年1月至2021年2月在台州市中医院因腰痛就诊的60例患者的腰椎CT平扫DICOM影像。采用OsiriX软件对其进行3D观察、测量L_(3)、L_(4)、L_(5)双侧椎弓根的横截面积、矢状径(TD)、冠状径(AP),并按照置钉方向测量每一椎弓根的理想置钉内收角和安全径深,统计结果数据,并通过与螺旋CT自带影像储存和传输系统(PACS)测量数据比较,来验证该软件测量的实用性。结果OsiriX软件与螺旋CT自带PACS系统测量的L_(3)、L_(4)、L_(5)双侧椎弓根TD与AP比较,差异均无统计学意义(均P>0.05)。结论OsiriX软件在下腰椎椎弓根形态学研究方面具有可行性,能够提供指导下腰椎椎弓根螺钉内固定手术决策的3D图像。Objective To explore the application value of DICOM image analysis software OsiriX in the morphological measurement of lower lumbar pedicle.Methods From January 2019 to February 2021,the DICOM images of lumbar plain CT scan of 60 patients with low back pain in Taizhou Traditional Chinese Medicine Hospital were collected.OsiriX image analysis software was used to conduct 3D observation,measure the cross-sectional area,transverse diameter(TD)and anteroposterior diameter(AP)of bilateral pedicle of L_(3),L_(4)and L_(5),and measure the ideal angle and safe diameter of each pedicle according to the direction of screw placement.The statistical results were compared with the measurement data of the spiral CT with PACS system to verify the practicability of the software measurement.Results There was no significant difference between TD and AP of L_(3),L_(4),L_(5)pedicle measured by OsiriX software and spiral CT with PACS system(all P>0.05).Conclusion OsiriX software has practical feasibility in the morphological study of lower lumbar pedicle,and it can provide 3D images to guide the surgical decision of lower lumbar pedicle screw fixation.
