急性期带状疱疹患者双侧颞叶海马区细胞代谢变化及与认知功能、疼痛程度的相关性

摘　　要：目的探讨急性期带状疱疹患者双侧海马区细胞代谢变化,并分析其与认知功能、疼痛程度的相关性。方法选取2020年7月至2021年12月在杭州市第三人民医院住院且发病1个月内的62例急性期带状疱疹患者(带状疱疹组)及55例体检志愿者(对照组)。均于入组72 h内采用简易精神状态评价量表(MMSE)、数字分级法(NRS)评估认知功能、疼痛强度,采用磁共振波谱(^(1)H-MRS)检测双侧海马区细胞代谢指标。分析带状疱疹患者MMSE、NRS评分与胆碱(Cho)/肌酸(Cr)、氮-乙酰天门冬氨酸(NAA)/Cr、NAA/Cho水平的相关性。结果两组患者MMSE评分比较差异无统计学意义(P>0.05);带状疱疹患者左、右侧海马区Cho/Cr水平明显低于对照组,差异均有统计学意义(均P<0.05),但NAA/Cr、NAA/Cho水平差异均无统计学意义(均P>0.05)。MMSE评分、NRS评分与双测海马区Cho/cr、NAA/Cr、NAA/Cho水平均无相关性(均P>0.05)。结论带状疱疹患者在急性期无明显认知功能障碍表现,但双侧海马区细胞代谢受到影响。Objective To investigate the changes of cell metabolism in bilateral hippocampus of patients with herpes zoster in acute phase.Methods ^(1)H-MRS(magnetic resonance spectroscopy)was used to detect the metabolism of cells in the bilateral hippocampus of 62 patients with acute herpes zoster admitted in Hangzhou Third People’s Hospital from July 2020 to December 2021,and 55 healthy subjects(control group).The mini-mental state examination(MMSE)and numerical rating scale(NRS)were used to evaluate the cognitive function and pain intensity of patients within 72 h after admission.The correlation of the NRS score with the duration of herpes zoster;and the correlation of MMSE and NRS scores with the levels of Cho/Cr,NAA/Cr and NAA/Cho were analyzed.Results There was no significant difference in the MMSE score between herpes zoster patients and control group(P>0.05).The level of choline(Cho)/creatine(Cr)in the left/right hippocampus of the herpes zoster patients was significantly lower than that of the control group(P<0.05);while there were no significant differences in N-acetylaspartate(NAA)/Cr and NAA/Cho levels between two groups(P>0.05).The NRS score was not correlated with the duration of disease(P>0.05);and there was also no significant correlation between MMSE score and levels of Cho/Cr,NAA/Cr and NAA/Cho in hippocampus(P>0.05).Conclusion Patients with herpes zoster may not have cognitive impairment in the acute stage,but the cell metabolism of bilateral hippocampus is affected.
