摘　　要：目的了解老年慢性病患者的基本情况及其生命质量状况,并进一步探究其生命质量的影响因素。方法收集2018年中国健康与养老追踪调查(CHARLS)中4278名65岁及以上老年人的数据资料,对老年人慢性病患病情况进行分析;采用欧洲五维健康量表(EQ-5D)对患者健康相关生命质量进行度量。结果2018年65岁及以上老年人的患病率达到76.18%,患有2种及以上慢性病的老人占41.91%,慢性病患者的健康效用值明显低于非慢性病患者;性别、受教育程度、睡眠时间、对生活的满意程度以及慢性病患病数是影响慢性病患者生命质量的关键因素。结论应着力提高基层医疗卫生资源可及性,重点关注女性、多种慢性病共存等人群,加大对于社区的政策和基金支持,健全社区养老服务,提升老年慢性病患者生命质量。Objective To explore the situation of elderly patients with chronic diseases and their quality of life and to find out the factors influencing their quality of life.Methods Data of a total of 4278 community residents aged over 65 years old were selected from The China Health and Retirement Longitudinal Study(CHARLS)for the study of the prevalence situation of elderly residents’developing chronic diseases.European Five-Dimensional Health Scale(EQ-5 D)was used to evaluate their quality of life.Results In 2018,the prevalence rate of elderly people aged 65 and over reached 76.18%,and the elderly with 2 or more chronic diseases accounted for 41.91%.The health utility value of people with chronic diseases was significantly lower than those without.The key factors affecting the quality of life of patients with chronic diseases were gender,educational background,sleep time,satisfaction with life and the number of chronic diseases.Conclusions Efforts should be made to improve the accessibility of primary medical and health resources,attach importance to population of women and those with coexistence of chronic diseases.Meanwhile,it is also important to increase policy and fund support for communities,improve community elderly care services to improve the quality of life of elderly population with chronic diseases.
