摘　　要：目的探讨常规器械行经脐单孔腹腔镜胆囊切除术(transumbilical single-port laparoscopic cholecystectomy,TSLC)的安全性和可行性。方法去回顾性分析2018年10月至2021年5月施行常规器械经脐单孔腹腔镜胆囊切除术93例的临床资料。结果93例手术均顺利完成,无一例中转开放手术。5例因胆囊三角解剖困难中转为二孔法腹腔镜胆囊切除术,1例胆源性胰腺炎保守治疗后中转为三孔法腹腔镜胆囊切除术,手术时间20~75min,平均(37.4±12.9)min;出血量2~10 ml,平均(2.8±1.5)ml。无胆管损伤、腹腔内出血、切口感染等手术并发症。术后住院2~6d,平均(3.4±0.9)d,随访时间3~33个月,手术瘢痕隐蔽,术后疼痛轻,患者非常满意。结论使用常规器械行TSLC对有经验的术者是安全可行的,而且术后疼痛轻,美容效果很好,值得临床有选择的推广。Objective To investigate the safety and feasibility of transumbilical single-port laparoscopic cholecystectomy(TSLC)with conventional instruments.Methods The clinical data of 93 cases of conventional instrument transumbilical single port laparoscopic cholecystectomy from October 2018 to May 2021 were retrospectively analyzed.ResultsAll 93 operations were successfully completed,and none of them were converted to open surgery.Five cases were converted to two-port laparoscopic cholecystectomy due to difficulty in dissection of the gallbladder triangle,and one case of biliary pancreatitis was converted to three-port laparoscopic cholecystectomy after conservative treatment.The operation time was 20 to 75 minutes,with an average of(37.4±12.9)min;bleeding volume is 2~10 ml,with an average of(2.8±1.5)ml.There was no operative complications such as bile duct injury,intra-abdominal hemorrhage,and incision infection.The postoperative hospitalization was 2 to 6 days,with an average of(3.4±0.9)days,and the follow-up time was 3 to 33 months.The surgical scar was hidden and the postoperative pain was mild.The patient was very satisfied.Conclusion The use of conventional equipment to perform TSLC is safe and feasible for experienced surgeons,and the postoperative pain is light and the cosmetic effect is very good.It is worthy of clinical promotion.
