化痰通络汤联合温针灸肩三针治疗脑卒中后肩手综合征痰瘀阻络证的临床观察

摘　　要：目的:观察化痰通络汤联合温针灸肩三针治疗脑卒中后肩手综合征(SHS)痰瘀阻络证的临床疗效。方法:将90例脑卒中后SHS痰瘀阻络证患者随机分为对照组和治疗组,每组45例。对照组采用常规疗法,治疗组在对照组基础上增加化痰通络汤与温针灸肩三针联合治疗,疗程为1个月。对比两组患者的疗效、中医证候积分、肩手综合征评估量表(SHSS)评分、上肢Fugl-Meyer功能量表(U-FMA)评分。结果:治疗组总有效率为86.67%(39/45),对照组总有效率为68.89%(31/45),治疗组高于对照组(P<0.05)。治疗后,两组患者中医证候积分、SHSS评分均较治疗前降低(P<0.05),U-FMA评分较治疗前升高(P<0.05),且治疗组患者中医证候积分、SHSS评分均低于对照组(P<0.05),U-FMA评分高于对照组(P<0.05)。结论:化痰通络汤联合温针灸肩三针治疗SHS痰瘀阻络证患者有较好疗效,能显著改善患者临床症状、肢体运动功能。Objective:To observe the clinical effects of Huatan Tongluo Decoction and Jiansanzhen acupoints in the treatment of shoulder-hand syndrome(SHS)after stroke of phlegm-stasis blocking collateral type.Methods:A total of 90 patients with SHS after stroke were randomly divided into control group and treatment group,with 45 cases in each group.The control group was treated with conventional therapy,and the treatment group was given Huatan Tongluo Decoction and warm acupuncture and moxibustion on Jiansanzhen acupoints on the basis of the control group.The course of treatment was 1 month.The efficacy,TCM syndrome score,shoulder-hand syndrome scale(SHSS)score and upper-limb Fugl-Meyer function scale(U-FMA)score of the two groups were compared.Results:The total effective rate of the treatment group was 86.67%(39/45),and that of the control group was 68.89%(31/45),which was higher in the treatment group than that in the control group(P<0.05).After treatment,the scores of TCM syndromes score and SHSS score of the two groups were lower than those before treatment(P<0.05),while the scores of U-FMA score was higher than that before treatment(P<0.05),the scores of TCM syndromes and SHSS of the treatment group were significantly lower than those of the control group(P<0.05),while the scores of U-FMA were significantly higher than those of the control group(P<0.05).Conclusion:Huatan Tongluo Decoction combined with Jiansanzhen acupoints for patients with SHS after stroke of phlegm-stasis blocking collateral type can significantly improve the clinical symptoms and motor function of the limbs,and has a good curative effect.
