静脉全麻下儿童寰枢关节半脱位手法治疗初步探讨

摘　　要：目的探讨静脉全麻下儿童寰枢关节半脱位手法治疗的效果。方法选取2017年10月—2021年10月期间在淄博市妇幼保健院住院治疗的60例诊断为寰枢关节半脱位患儿,随机分为牵引治疗组和手法治疗组,各30例。牵引治疗组患者仅接受传统的牵引疗法,而手法治疗组则在牵引治疗组的基础上同时给予手法治疗,对比两组的有效率和疼痛评分。结果手法治疗组的总有效率(96.67%)高于牵引治疗组(73.33%),差异有统计学意义(χ^(2)=4.706,P=0.030)。治疗前,两组NPQ评分比较,差异无统计学意义(P>0.05);治疗后,两组NPQ评分均有不同程度的下降,手法治疗组下降更加明显,且低于牵引治疗组,差异有统计学意义(P<0.05)。结论静脉全麻下手法治疗儿童寰枢关节半脱位具有较高的临床可行性,能够有效缓解患儿的临床症状,维持寰枢关节的正常结构与功能,同时还有利于减轻患儿疼痛感。Objective To explore the manipulative treatment of atlantoaxial subluxation in children under intravenous general anesthesia.Methods 60 children diagnosed with atlantoaxial subluxation who were hospitalized in Zibo Maternal and Child Health Hospital from October 2017 to October 2021 were selected,they were randomly divided into the traction treatment group and the manipulation treatment group,with 30 cases in each.The patients in the traction treatment group received only traditional traction therapy,while the patients in the manipulation treatment group were given manipulation therapy on top of the traction treatment group,and the efficiency and pain scores of the two groups were compared.Results The overall effective rate of the manipulative treatment group(96.67%)was higher than that of the traction treatment group(73.33%),and the difference was statistically significant(χ^(2)=4.706,P=0.030).Before treatment,there was no statistically significant different in NPQ scores between the two groups(P>0.05);after treatment,the NPQ scores of both groups decreased to different degrees,and the decrease in the manipulative treatment group was more obvious and lower than that in the traction treatment group,and the difference was statistically significant(P<0.05).Conclusion The treatment of atlantoaxial subluxation in children under intravenous general anesthesia is clinically feasible and can effectively relieve the clinical symptoms of children and maintain the normal structure and function of the atlantoaxial joint,and also help to reduce the pain of children.
