摘 要:目的 研究超声内镜弹性成像联合声速匹配技术对胆系及壶腹周围疾病的诊断价值。资料与方法 回顾性分析2018年11月—2020年9月于酒泉市人民医院手术确诊的22例胆系及壶腹周围疾病患者,所有患者均行超声内镜弹性成像联合声速匹配技术检查、CT及MRI检查。以病理结果为金标准,分析3种检查方式对胆系及壶腹周围疾病患者病变性质的检出率和诊断效能。结果 22例患者术后病理结果显示阳性18例(均为肿瘤)、阴性4例(为炎症或正常)。CT检出阳性11例(50.0%),MRI检出阳性13例(59.1%),超声内镜弹性成像联合声速匹配技术检查检出阳性19例(86.4%),超声内镜弹性成像联合声速匹配技术对胆系及壶腹周围疾病的检出率明显高于CT和MRI,差异有统计学意义(χ^(2)=6.705、4.125,均P<0.05)。超声内镜弹性成像联合声速匹配技术诊断胆系及壶腹周围疾病的敏感度和准确度分别为94.4%和86.4%,均高于CT(50.0%、50.0%)和MRI(61.1%、59.1%),差异有统计学意义(χ^(2)=0.003、0.010,0.016、0.042,P<0.05);3种技术的特异度、阳性预测值和阴性预测值差异均无统计学意义(均P>0.05)。结论 超声内镜弹性成像联合声速匹配技术诊断胆系及壶腹周围疾病的良恶性和病灶定位具有较高价值,值得临床应用及推广。Purpose To explore the clinical value of endoscopic ultrasound elastography plus sound speed correction on diagnosis of biliary system diseases and peripheral ampulla diseases. Materials and Methods 22 patients with biliary system diseases and peripheral ampulla diseases admitted from November 2018 to September 2020 in Jiuquan People’s Hospital were chosen as research subjects. The endoscopic ultrasound elastography plus sound speed correction, CT and MRI were performed. According to pathological examination results, the diagnosis efficacy of biliary system diseases and peripheral ampulla diseases based on three examination methods was compared.Results The number of positive, negative diseases were 18(tumors) and 4(inflammation or normal). CT revealed 11 positive diseases(50.0%), MRI showed 13 positive diseases(59.1%), and endoscopic ultrasound elastography plus sound speed correction revealed 19positive diseases(86.4%). The detection rate of biliary system diseases and peripheral ampulla diseases based on endoscopic ultrasound elastography plus sound speed correction was significantly higher than that of CT and MRI(χ^(2)=6.705, 4.125, all P<0.05);the diagnosis sensitivity and accuracy based on endoscopic ultrasound elastography plus sound speed correction(94.4%, 86.4%) were significantly higher than those of CT(50.0%, 50.0%) and MRI(61.1%, 59.1%;χ^(2)=0.003, 0.010, 0.016, 0.042;all P<0.05);the diagnosis specificity, positive predictive value and negative predictive value based on three examination methods were not significantly different(P>0.05). Conclusion The endoscopic ultrasound elastography combined with sound speed correction have better effects for the diagnosis of biliary system diseases and peripheral ampulla diseases. It is worthy of clinical application and promotion.
