摘　　要：目的 探讨经会阴超声实时剪切波弹性成像在孕晚期用于预测早产风险的价值。资料与方法 回顾性纳入2018年1月—2020年12月在石家庄市第四医院进行产前检查的早产孕妇42例、正常分娩孕妇93名,采用经会阴超声技术,测量其宫颈长度及宫颈内口、外口弹性模量值。分析宫颈长度、宫颈内口与外口弹性模量值、年龄、孕龄相关指标与早产的关联性,采用受试者工作特征曲线评价相关指标的应用效能。结果 与正常分娩孕妇相比,早产孕妇宫颈长度较短(3.41±0.91比2.89±0.52;t=3.481,P<0.001)、宫颈内口弹性模量值较低(8.14±2.60比6.14±3.00;t=3.938,P<0.001)。宫颈长度和宫颈内口弹性模量值曲线下面积分别为0.773、0.758(P均<0.001)。结论 经会阴超声获取得到的孕晚期孕妇宫颈长度和宫颈内口弹性模量值对早产具有较好的预测价值。Purpose To assess the application value of transperineal ultrasound real-time shear wave elastography in assessing the risk of preterm birth in pregnant women in late trimester. Materials and Methods A total of 135 pregnant women, including 42women with preterm birth and 93 women with normal labor, received prenatal examination at The Fourth Hospital of Shijiazhuang from January 2018 to December 2020 were retrospectively included. Cervical length(CL) and stiffness(internal cervical and external cervical) were measured by transperineal ultrasound. Relativity between preterm labor and CL, stiffness, age, weeks were accessed.Receiver operating characteristic(ROC) curve was established for accessing efficacy of relative data. Results CL of study group was significantly shorter than that of control group(3.41±0.91 vs. 2.89±0.52;t= 3.481, P<0.001). The stiffness of internal(8.14±2.60vs. 6.14±3.00;t= 3.938, P<0.001) cervix was higher in control group than study group. It was observed for ROC that, the area under curve of CL and internal stiffness were 0.773, 0.758 relatively both with P<0.001. Conclusion Cervical tissue length and elasticity indexes obtained by transperineal can be used to assess the risk of preterm birth feasibly.
