摘  要:目的 探讨产前运用超声时间-空间关联成像(STIC)联合高分辨率血流显像诊断脐带异常的应用价值。资料与方法 收集2019年2月—2021年6月于安徽省妇幼保健院产前疑脐带异常的11例胎儿,分别进行高分辨率血流显像及STIC数据采集,并与产后结果对比。结果 产前运用STIC联合高分辨率血流显像共诊断11例脐带异常,其中2例为单绒毛膜单羊膜双胎儿脐带缠绕,2例为脐带真结;7例为脐带扭转;11例中2例脐带扭转,但二维及彩色血流显像出现漏诊,STIC联合高分辨率血流显像做出修正;二维超声漏诊脐带真结1例及双胎脐带缠绕1例,STIC联合高分辨率血流显像做出修正。11例中9例出生后证实,2例引产后证实。结论 STIC联合高分辨率血流显像可以更直观及立体地显示脐带血管的空间位置关系,有助于胎儿脐带异常的产前诊断。Purpose To explore the application value of spatio-temporal image correlation(STIC) combined with high definition(HD live flow imaging in prenatal diagnosis of umbilical cord abnormalities. Materials and Methods A prospective study was conducted on fetuses with suspected umbilical cord abnormalities during the period from February 2019 to June 2021 in Anhui Provincial Maternity and Child Health Hospital. High definition live flow imaging and STIC data were collected and compared with the postpartum results. Results Eleven cases of umbilical cord abnormalities were diagnosed by STIC combined with high definition live flow imaging before delivery. Among them, two cases were monochorionic monoamniotic membrane twins with umbilical cord entanglement;two cases were true knot of umbilical cord. There were seven cases of umbilical cord torsion. In 11 cases, two cases of umbilical cord torsion were missed by two-dimensional and color flow imaging, which were corrected by STIC+HD live flow. One case of true knot of umbilical cord and one case of umbilical cord entanglement of twins were missed by two-dimensional ultrasound, which were corrected by STIC+HD live flow. Among the 11 cases, 9 cases were confirmed after birth, two cases were confirmed after induction of labor. Conclusion STIC combined with high definition live flow imaging can more intuitively and stereoscopically display the spatial relationship of umbilical cord vessels, which is helpful for prenatal diagnosis of fetal umbilical cord abnormalities.
