摘　　要：分析室早患者心电图特点,总结能否在消融术前有效预测室早起源。方法:选取55例室早射频消融术的患者,右室流出道组(RVOT)39例,左室流出道组(LOVT)16例,术前均完善心电图检查,对多项指标测量分析,将术中成功消融靶点认为是室早起源。回顾性分析并计算室早患者基线资料,V2移行指数、移行区指数、SV2/RV3指数。结果:两组患者基线资料未见差异,三种标测方法均能在术前预测室早起源,其测量值两组有显著差异,移行区指数和SV2/RV3较V2移行指数在预测RVOT室早特异性更佳。结论术前根据心电图预测室早左右室起源能够更好帮助术者在术中判定室早起源点。Objective:To explore the ECG characteristics of premature ventricular contraction,and differentiating between left and right OT-VA origins before patients underwent raiofrequency catheter ablation.Methods:Retrospective analysis was performed on the figures of 55 successfully ablated cases with right ventricular outflow tract(RVOT,n=39)PCV or left ventricular outflow tract(LVOT,n=16)PCV.Standard 12-lead ECG were recorded before the ablation,and measure and analysis several multiple indicators simultaneously.The target of successful intraoperative ablation is defined as the origin of premature ventricular contractions.Retrospective analysis and calculation of V2 transition index,transition zone index,SV2/RV3 index in patients with ventricular premature contractions.Results:There were no significant differences between the RVOT and LVOT groups with baseline data.The V2 Transition Index,SV2/RV3,Transition Zone Index,all three methods can predict the origin of the premature ventricular contractions preoperatively,and the measurements are significantly different in the origin of the left and right outflow tracts。And the transition zone index and SV2/RV3 are more accurate than the V2 transition index in predicting the positions of the premature ventricular contraction.Conclusion:Predicting the origin of the left and right outflow tract based on the electrocardiogram before the operation can help the surgeon to determine the origin of the premature ventricular contraction during radiofrequency ablation.
