摘 要:目的探讨影像学手段(X线检查、MRI检查、CT检查)在颈椎病中的诊断价值。方法选取唐山市第二医院在2018年11月至2021年8月住院治疗的颈椎病患者343例(脊髓型颈椎病29例、交感型颈椎病117例、神经根型颈椎病83例、椎动脉型颈椎病41例、混合型颈椎病73例),接受影像学检查(X线检查、MRI检查、CT检查),并比较影像学手段(X线检查、MRI检查、CT检查)的诊断价值。结果对于脊髓型颈椎病进行诊断时,MRI诊断准确率最高为96.6%,其次为CT,诊断准确率89.7%,X线诊断准确率最低为65.5%。对于交感型颈椎病进行诊断时,MRI诊断准确率最高为95.7%,其次为CT,诊断准确率86.3%,X线诊断准确率最低为67.5%。对于神经根型颈椎病进行诊断时,MRI诊断准确率最高为95.2%,其次为CT,诊断准确率88.0%,X线诊断准确率最低为63.9%。对于椎动脉型颈椎病进行诊断时,MRI诊断准确率最高为95.1%,其次为CT,诊断准确率80.5%,X线诊断准确率最低为68.3%。对于混合型颈椎病进行诊断时,MRI诊断准确率最高为94.5%,其次为CT,诊断准确率83.6%,X线诊断准确率最低为64.4%。X线对不同颈椎疾病的诊断准确率与MRI、CT比较差异均有统计学意义(P<0.05)。结论颈椎病MRI诊断价值最高,其次为CT和X线片检查。Objective To explore the diagnostic value of imaging methods[X-ray,magnetic resonance imaging(MRI)and computed tomography(CT)]in cervical spondylosis(CS).Methods Totally 343 hospitalized patients with CS included 29 patients with cervical spondylotic myelopathy(CSM),117 patients with sympathetic cervical spondylosis(SCS),83 patients with cervical spondylotic radiculopathy(CSR),41 patients with vertebral artery type of cervical spondylosis(VACS),73 patients with mixed cervical spondylosis(MCS)these patients visited Tangshan Second Hospital from November 2018 to August 2021.imageological examination(X-ray,MRI,CT)were conducted in patients,and the diagnostic value of imaging methods(X-ray,MRI and CT)were included as comparator.Results The diagnostic accuracy for CSM of MRI examination was maximum(96.6%),followed by CT(89.7%),and X-ray was minimum(65.5%).The diagnostic accuracy for SCS of MRI,CT,X-ray was 95.7%,86.3%,67.5%,respectively.The diagnostic accuracy for CSR was 95.2%,88.0%,63.9%for MRI,CT,X-ray.The diagnostic accuracy for CSA of 95.1%,80.5%,68.3%,respectively.The diagnostic accuracy for MAS of the above threes 94.5%vs 83.6%vs 64.4%.For VACS,the diagnostic accuracy of X-ray was inferior to MRI and CT(P<0.05).Conclusion MRI have maximum diagnostic valuefor CS followed by CT and X-ray.
