家庭群组式产后健康教育促进初产妇母乳喂养的效果研究

摘　　要：目的探讨家庭群组式产后健康教育在初产妇母乳喂养延续性健康教育中的应用效果。方法将100位研究对象按住院号奇偶数分为对照组和试验组。对照组给予常规母乳喂养健康教育方案,试验组给予家庭群组式产后健康教育方案。在产后7 d、产后42 d及产后6个月时分别评估2组产妇母乳喂养知识掌握情况、母乳喂养自我效能得分及喂养方式,以及健康教育前后2组产妇主观幸福感及家属产后支持水平评分。结果产后7 d、42 d及6个月与产妇母乳喂养知识得分的交互作用差异有统计学意义(P<0.05),与产妇母乳喂养自我效能得分交互作用差异有统计学意义(P<0.05);2组产妇42 d时喂养方式间差异无统计学意义(P>0.05),产后6个月时喂养方式差异有统计学意义(P<0.05);实施后试验组产妇主观幸福感评分及家属产后支持水平评分均显著高于对照组,差异有统计学意义(P<0.05)。结论家庭群组式产后健康教育有助于初产妇及家属掌握母乳喂养知识和技能,增强初产妇母乳喂养自我效能,提升主观幸福感,进而提高6个月内母乳喂养率。Objective To explore the effect of family group postpartum health education in continuous primiparas breastfeeding.Methods Subjects(n=100)were grouped as the odevity of hospitalization number,these patients were assigned to receive either routine breastfeeding education(control group)or family group postpartum health education(observation group).The maternal knowledge of breastfeeding,breastfeeding self-efficacy(BSE)scores and feeding methods on days 7,42 and month 6 of intervention were included as comparators,as well as maternal subjective well-being and postpartum support level from their families before and after of intervention were evaluated.Results The interaction between postpartum 3 time points and maternal breastfeeding knowledge score was significant difference(P<0.05),but no significant difference in BSE(P<0.05).There was no significant difference in feeding methods on day 42 between groups(P>0.05),but significant difference at month 6(P<0.05).After implementation,the scores of maternal subjective well-being and postpartum support level of family members in the observation group were superior to those in the control group(P<0.05).Conclusion Family group postpartum health education can help primiparas and their families to master the knowledge and skills of breastfeeding,enhance BSE,improve subjective well-being,with better breastfeeding rate within 6 months.
