摘  要:目的研究低浓度罗哌卡因连续硬膜外阻滞对妊娠期高血压疾病患者血浆内皮素-1(ET-1)与一氧化氮(NO)水平的影响。方法选择符合妊娠期高血压疾病(HDCP)诊断标准,且确定施行剖宫产手术的患者60例,按住院顺序随机分为观察组与对照组,每组30例。观察组在对症治疗的同时行低浓度罗哌卡因连续硬膜外阻滞,对照组仅行对症治疗。2组患者均在同时段内检测ET-1与NO的浓度。结果观察组阻滞后血浆ET-1浓度与对照组第2次血浆ET-1浓度比较明显降低,差异有统计学意义(P<0.05)。观察组硬膜外阻滞后血浆NO浓度与对照组患者第2次NO浓度比较明显提高,差异有统计学意义(P<0.05)。结论连续硬膜外阻滞后可使HDCP患者血浆ET-1浓度明显降低,血浆NO浓度显著增高,其结果为低浓度罗哌卡因连续硬膜外阻滞辅助治疗HDCP提供了依据。Objective To study the effect of continuous epidural block(CEB)with low concentration ropivacaine on endothelin-1(ET-1)and nitric oxide(NO)in pregnant women with hypertensive disorder complicating pregnancy(HDCP).Methods HDCP pregnant women(n=60)underwent cesarean operation were randomized 1∶1 to receive symptomatic treatment(control group)or CEB with low concentration ropivacaine(observation group).The aim was to detect the concentration of ET-1 and NO.Results The concentration of ET-1 after epidural block in the observation group was significantly lower than that at second times in the control group(P<0.05);NO after epidural block in the observation group was significantly higher than that at second times in the control group(P<0.05).Conclusion CEB can significantly decrease ET-1 and increase NO in the pregnant women with HDCP.The result provides evidence for adjuvant treatment of HDCP by CEB with low concentration ropivacaine.
