云手加外展架预防肱骨近端骨折术后功能障碍

摘　　要：目的探讨云手结合定时定角度外展架固定康复锻炼对预防肱骨近端骨折术后肩关节功能障碍的疗效。方法回顾性分析2018年1月至2020年1月收治的86例肱骨近端骨折患者的临床资料,将肱骨近端骨折切开复位内固定术后3周内予以云手加定时定角度外展架固定康复锻炼,3周后行常规康复方案加云手作为观察组,将肱骨近端骨折切开复位内固定术后3周内患肩处于自主体位,3周后行常规康复方案作为对照组。将2组患者骨质疏松、骨折愈合时间、肩关节功能恢复时间、肩关节功能恢复程度作对比性研究分析。结果观察组在肩关节功能恢复时间及肩关节功能恢复程度上明显优于对照组,差异有统计学意义(P<0.05),2组骨质疏松、骨折愈合时间比较差异无统计学意义(P>0.05)。结论肱骨近端骨折术后采取云手加定时定角度外固定康复治疗,能有效地促进肩关节功能恢复,缩短恢复时间,减少并发症的发生率,值得临床推广应用。Objective To investigate the effect of rehabilitation exercise with cloud hand+timing set point of external fixation in preventing postoperative shoulder dysfunction of proximal humerus fracture(PHF).Methods The retrospective study included 86 patients with PHF visited our department from January 2018 to January 2020.During postoperative period of open reduction internal fixation(ORIF),the patients were randomly assigned to receive either active position mode for shoulder(control group)or cloud hand+timing set point of external fixation(observation group)for 3 weeks.The intergroup gender,age,fracture healing time,recovery time and extent of shoulder joint function were included as comparators.Results The recovery time and extent of shoulder joint function in the observation group were notably superior to the control group(P<0.05).The difference was not statistically significant in the gender,age,fracture healing time between groups(P>0.05).Conclusion For patients with PHF,cloud hand+timing set point of external fixation can effectively promote postoperative recovery of shoulder joint function,shorten the recovery time and reduce complications,and worthy of clinical application.
