中频电治疗促进前交叉韧带重建术后早期康复的效果研究

摘　　要：目的:在前交叉韧带重建后的早期康复阶段,引入基于中频电治疗的康复方案,探索与传统下肢锻炼相比,中频电治疗带来的额外康复效果。方法:选取昆明医科大学第一附属医院2020年6月-2021年12月收治的行前交叉韧带(ACL)重建术的52例患者作为研究对象,随机分为对照组(n=26)和中频组(n=26)。对照组实施术后常规康复治疗;中频组在对照组的基础上实施中频电治疗。采用视觉模拟评分法(VAS)评估术后1、3、7、14 d的疼痛感受。测量膝围评估膝关节肿胀程度,测量大腿围评估肌肉萎缩恢复情况,通过等速肌力评估股四头肌力量,通过国际膝关节文献委员会膝关节评估表(IKDC)评估膝关节功能恢复情况。结果:两组术后1 d VAS评分比较,差异无统计学意义(P>0.05);术后3、7、14 d,中频组VAS评分均显著低于对照组,差异均有统计学意义(P<0.05)。术后7 d,中频组膝围差明显小于对照组(P<0.05);术后14 d,两组膝围差比较,差异无统计学意义(P>0.05)。术后4、8周,中频组腿围差、股四头肌力量差值比、IKDC评分均明显优于对照组(P<0.05)。结论:中频电治疗可以有效地缓解ACL术后早期的膝关节疼痛与肿胀,促进股四头肌围度增长,恢复股四头肌力量,值得进一步临床推广与应用以加速ACL重建后的早期康复。Objective: In the early rehabilitation stage after anterior cruciate ligament reconstruction,the rehabilitation scheme based on medium frequency electrotherapeutic is introduced to explore the additional rehabilitation efficacy brought by the medium frequency electrotherapeutic compared with the traditional lower limb exercise. Method: A total of 52 patients who underwent anterior cruciate ligament(ACL) reconstruction in the First Affiliated Hospital of Kunming Medical University from June 2020 to December 2021 were selected as the research subjects and randomly divided into a control group(n=26) and an medium frequency group(n=26). The control group was treated routine rehabilitation after operation, and medium frequency group was treated with medium frequency electrotherapeutic on the basis of the control group.Visual analogue scale(VAS) was used to evaluate the pain sensation on the 1, 3, 7 and 14 d after operation. Knee circumference was measured to evaluate the swelling degree of knee joint, thigh circumference was measured to evaluate the recovery of muscle atrophy, the strength of quadriceps femoris was evaluated by isokinetic muscle strength, and the functional recovery of knee joint was evaluated by the international knee documentation committee knee evaluation form(IKDC). Result: 1 d after operation, there was no significant difference in VAS scores between the two groups(P>0.05);3, 7, and 14 d after operation, the VAS scores in the medium frequency group were significantly lower than those in the control group, the differences were statistically significant(P<0.05). 7 d after operation, the knee circumference difference in the medium frequency group was significantly smaller than that in the control group(P<0.05);14 d after operation, there was no significant difference in knee circumference between the two groups(P>0.05). 4, 8 weeks after operation, the thigh circumference difference,quadriceps femoris strength difference ratio and IKDC score in the medium frequency group were significantly better tha
