MSCT多平面重建联合静脉肾盂造影在输尿管阴性小结石中的诊断分析

摘　　要：目的分析多层螺旋CT(MSCT)多平面重建联合静脉肾盂造影(IVP)在输尿管阴性小结石(UNSS)中的诊断价值。方法选取2017年1月至2021年12月我院疑似UNSS患者60例,均采用MSCT平扫与MSCT多平面重建联合IVP检查,以输尿管检查为“金标准”,统计两种检查方式的诊断结果和诊断效能,比较结石检出率。结果本组60例疑似UNSS患者中,经输尿管检查确诊阳性52例,阴性8例;MSCT平扫诊断出真阳性39例,真阴性5例;MSCT多平面重建联合IVP诊断出真阳性51例,真阴性7例;MSCT多平面重建联合IVP诊断灵敏度98.08%(51/52)、准确度96.67%(58/60)较MSCT平扫75.00%(39/52)、73.33%(44/60)高,漏诊率1.92%(1/52)较MSCT平扫25.00%(13/52)低(P<0.05);52例UNSS患者中,经MSCT平扫检测出48个结石,输尿管上段、中段、下段分别为14个、18个、16个,MSCT多平面重建联合IVP检查共检测出75个结石,输尿管上段、中段、下段分别为31个、19个、25个,MSCT多平面重建联合IVP检测输尿管上段结石检出率41.33%(31/75)较MSCT平扫29.17%(14/48)高(P<0.05)。结论MSCT多平面重建联合IVP诊断UNSS可提升诊断灵敏度和准确度,显著减少漏诊情况的发生,且二者联合能提升结石检出率,值得临床推广使用。Objective To analyze the diagnostic value of multiplanar reconstruction of multi-slice spiral CT(MSCT)combined with intravenous pyelography(IVP)in ureteral negative small stones(unss).Methods 60 patients with suspected unss in our hospital from January 2017 to December 2021 were selected.They were examined by MSCT plain scan and MSCT multiplanar reconstruction combined with IVP.Taking ureter examination as the"gold standard",the diagnostic results and diagnostic efficacy of the two examination methods were counted,and the stone detection rate was compared.Results among the 60 suspected unss patients,52 cases were positive and 8 cases were negative by ureteral examination;MSCT plain scan showed true positive in 39 cases and true negative in 5 cases;MSCT multiplanar reconstruction combined with IVP diagnosis was true positive in 51 cases and true negative in 7 cases;The sensitivity and accuracy of MSCT multiplanar reconstruction combined with IVP were 98.08%(51/52)and 96.67%(58/60)higher than those of MSCT plain scan 75.00%(39/52)and 73.33%(44/60),and the missed diagnosis rate was 1.92%(1/52)lower than that of MSCT plain scan 25.00%(13/52)(P<0.05);In 52 patients with unss,48 stones were detected by MSCT plain scan,14,18 and 16 in the upper,middle and lower segments of ureter respectively.75 stones were detected by MSCT multiplanar reconstruction combined with IVP,and 31,19 and 25 in the upper,middle and lower segments of ureter respectively.The detection rate of stones in the upper segment of ureter by MSCT multiplanar reconstruction combined with IVP was 41.33%(31/75),which was higher than that of 29.17%(14/48)by MSCT plain scan(P<0.05).Conclusion MSCT multiplanar reconstruction combined with IVP in the diagnosis of unss can improve the diagnostic sensitivity and accuracy,significantly reduce the occurrence of missed diagnosis,and the combination of the two can improve the detection rate of stones,which is worthy of clinical application.
