泄浊通络方联合穴位埋线治疗痛风性肾病

摘　　要：目的:观察泄浊通络方联合穴位埋线治疗痛风性肾病的临床疗效。方法:66例痛风性肾病按照随机数字表法分为观察组和对照组各33例。对照组给予非布司他片治疗,观察组在对照组治疗基础上给予泄浊通络方联合穴位埋线治疗。两组均以4周为1个疗程,均治疗6个疗程。治疗后比较两组患者临床疗效,中医证候积分,血肌酐(serum creatinine,Scr)、血尿酸(uric acid,UA)、24小时尿蛋白定量(24h-urine protein,24hUPr)、血沉(erythrocyte sedimentation rate,ESR)、超敏C反应蛋白(hypersensitive C-reactive protein,Hs-CRP)、胱抑素C(cystatin C,Cys C)等水平以及不良反应的发生情况。结果:①观察组有效率为96.97%(32/33),对照组有效率为78.79%(26/33),两组有效率比较,差异有统计学意义(P<0.05)。②观察组治疗后Scr[(90.71±18.79)μmol·L^(-1)]、UA[(426.7±44.12)mmol·L^(-1)]、24UPr[(0.87±0.55)g]、ESR[(19.35±4.66)mm·h^(-1)]、Hs-CRP[(7.13±1.79)mg·L^(-1)]、Cys C[(0.76±0.11)mg·L^(-1)]等指标水平均显著低于治疗前,且低于同期对照组(P<0.05)。③观察组治疗后各中医证候积分[关节肿痛(1.17±0.09)分、屈伸不利(1.27±0.46)分、筋骨疼痛(1.20±0.55)分、口干不欲饮(0.78±0.55)分、倦怠乏力(0.93±0.59)分、烦躁不安(1.22±0.38)分]均显著低于本组治疗前,且低于同期对照组(P<0.05)。④两组患者不良反应发生率分别为5.37%、2.82%,差异无统计学意义(P>0.05)。结论:泄浊通络方联合穴位埋线治疗痛风性肾病疗效显著,可显著改善患者肾功能,降低中医证候积分,且安全性高。Objective:To observe the clinical effect of Xiezhuo Tongluo Formula combined with acupoint catgut embedding in treatment of gouty nephropathy.Methods:66 cases of gouty nephropathy were randomly divided into observation group and control group,33 cases in each group.The control group was treated with febuxostat tablets,and the observation group was treated with Xiezhuo Tongluo Formula combined with acupoint catgut embedding on the basis of the control group.Both groups were treated for 6 courses with 4 weeks as a course of treatment.After treatment,the clinical efficacy and TCM syndrome scores of the two groups were compared.Serum creatinine(Scr),uric acid(UA),24-hour urinary protein(24hUPr),erythrocyte sedimentation rate(ESR),hypersensitive C-reactive protein(Hs-CRP),serum cystatin C(Cys C)and other levels and the occurrence of adverse reactions.Results:①The effective rate of the observation group was 96.97%(32/33),and the effective rate of the control group was 78.79%(26/33).②After treatment,the levels of Scr[(90.71±18.79)μmol·L^(-1)],UA[(426.7±44.12)mmol·L^(-1)],24 UPr[(0.87±0.55)g],ESR[(19.35±4.66)mm·h^(-1)],Hs-CRP[(7.13±1.79)mg·L^(-1)]and Cys C[(0.76±0.11)mg·L^(-1)]in the observation group were significantly lower than those before treatment,and lower than those in the control group(P<0.05).③After treatment,the scores of TCM syndromes in the observation group[joint swelling and pain(1.17±0.09)points,unfavorable flexion and extension(1.27±0.46)points,muscle and bone pain(1.20±0.55)points,dry mouth(0.78±0.55)points,fatigue(0.93±0.59)points,irritability(1.22±0.38)points]were significantly lower than those before treatment,and lower than those in the control group(P<0.05).④The incidence of adverse reactions in the two groups was 5.37%and 2.82%,respectively,and the difference was not statistically significant(P>0.05).Conclusion:Xiezhuo Tongluo Formula combined with acupoint catgut embedding is effective in the treatment of gouty nephropathy,which can significantly improve the renal functi
