鼻内镜联合低温等离子射频消融腺样体切除术治疗小儿鼾症的效果及对睡眠质量的影响

摘　　要：目的:探讨鼻内镜联合低温等离子射频消融腺样体切除术治疗小儿鼾症的效果及对睡眠质量的影响。方法:选取2021年1月至2022年1月泉州市儿童医院收治的小儿鼾症患儿102例作为研究对象,按照随机数字表法随机分为对照组和观察组,每组51例。对照组患儿行扁桃体摘除术和鼻镜腺样体切除术治疗,观察组患儿行鼻内镜联合低温等离子射频消融切除术治疗。比较2组患儿手术指标、治疗效果、并发症发生率、睡眠质量、呼吸情况。结果:观察组手术时间短于对照组,术中出血量少于对照组,住院时间短于对照组,2组比较差异均有统计学意义(均P<0.05)。观察组治疗总有效率为94.12%,对照组为78.43%,2组比较差异有统计学意义(P<0.05)。观察组手术并发症发生率为3.92%,对照组发生率为15.69%,2组比较差异有统计学意义(P<0.05)。治疗前,2组患儿匹兹堡睡眠质量指数(PSQI)评分、呼吸暂停-低通气指数、吞咽疼痛评分比较,差异均无统计学意义(均P>0.05);治疗后观察组PSQI评分、呼吸暂停-低通气指数、吞咽疼痛评分均低于对照组,2组比较差异均有统计学意义(均P<0.05)。结论:开展鼻内镜联合低温等离子射频消融腺样体切除术治疗小儿鼾症可取得满意的治疗效果,能有效减少手术创伤,缩短康复时间,降低并发症风险,显著改善睡眠质量和通气功能。Objective:To investigate the effect of nasal endoscopy combined with low temperature plasma radiofrequency ablation adenoidectomy in the treatment of snoring in children and its effect on sleep quality.Methods:A total of 102 children with snoring were selected and treated from January 2021 to January 2022.The children were randomly divided into two groups according to the random number table,with 51 cases in each group.The children in the control group were treated with tonsillectomy and endoscopic adenoidectomy.The children in the observation group were treated with nasal endoscopy combined with low temperature plasma radiofrequency ablation.The operative indexes,therapeutic effects,complications,sleep quality and respiration were compared between the two groups.Results:The operation time of the observation group was shorter than that of the control group,the amount of intraoperative bleeding was less than that of the control group,and the hospital stay was shorter than that of the control group.The difference between the two groups was statistically significant(P<0.05).The total effective rate of the observation group was 94.12%,that of the control group was 78.43%,The difference between the two groups was statistically significant(P<0.05).The incidence of surgical complications was 3.92%in the observation group and 15.69%in the control group and lower in the observation group.The difference between the two groups was statistically significant(P<0.05).There was no significant difference in Pittsburgh Sleep Quality Scale(PSQI),apnea hypopnea index and swallowing pain score between the two groups before treatment(P>0.05).After treatment,PSQI,apnea hypopnea index and swallowing pain score in the observation group were lower than those in the control group(P s<0.05).Conclusion:Nasal endoscopy combined with low temperature plasma radiofrequency ablation adenoidectomy in the treatment of snoring in children can achieve satisfactory therapeutic effect,effectively reduce surgical trauma,shorten rehabilitation time,redu
