摘 要:目的结核病是一种由结核分枝杆菌复合群引起的人畜共患慢性传染病。本研究通过调查四川省洪雅县奶牛场工作人员及奶牛结核分枝杆菌感染情况,为结核病防控提供建议。方法2018年对洪雅县奶牛场378名工作人员进行结核病检查,对奶牛场所有奶牛(10224头)进行结核菌素皮内变态反应试验,培养疑似结核病患者痰标本、结核菌素试验阳性牛及密接牛的食道-咽部分泌物和阳性牛所在环境样本,并使用PCR扩增检测纯培养物16s rDNA、hsp64、rpoB和16s-23s rRNA间隔区,通过测序比对鉴定纯培养物菌型。结果1名工作人员被诊断为肺结核;99头奶牛为结核菌素试验阳性,阳性率0.97%;培养分离菌株21株,经鉴定均为非结核分枝杆菌,包括18株M.nonchromogenicum,1株M.hiberniae,1株M.arupense和1株M.chitae。结论本研究未从奶牛中检出结核分枝杆菌,经鉴定,结核菌素试验阳性牛感染的菌株为非结核分枝杆菌,使用结核菌素试验检测牛结核感染的方法准确度受限,建议对阳性奶牛采用更准确的检测方法进行复检。Objective Tuberculosis was a zoonotic chronic infectious disease caused by mycobacterium tuberculosis complex(MTBC).The aim of our study was to investigate mycobacterium tuberculosis infection between bovine and humans in dairy farms in Hongya County of Sichuan Province,and to provide advice for tuberculosis prevention and control.Methods All 378 workers from dairy farms in Hongya County were screened for tuberculosis symptoms and examined.Skin tests for purified protein derivatives(PPD)were performed on all cows(n=10224).Sputum samples from suspected TB patients,Esophageal-pharyngeal(OP)secretions from PPD positive cows and their close-coupled cows,and environmental samples were cultured using modified L-J medium,then the pure cultures were tested by PCR to detect 16s rDNA,hsp64,rpoB and 16s-23s r RNA spacer region,sequencing comparison method was used to identify pure culture bacterial type.Results One worker was diagnosed tuberculosis.In total,99 cows showed PPD positive(0.97%),and 21isolates were obtained by culture.BLAST analysis classified these isolates as nontuberculous mycobacteria(NTM)(18 M.nonchromogenicum,1 M.hiberniae,1 M.arupense,and 1 M.chitae).Conclusions Mycobacterium tuberculosis was not detected in dairy cows in this study,PPD test positive cattle were identified as infected with non-tuberculous mycobacterium.The method used to detect TB infection in cows using PPD was not very accurate and a more accurate method is recommended.
