摘 要:目的探讨甲状腺素在正常范围内的2型糖尿病(type 2 diabetes mellitus,T2DM)患者的甲状腺素及甲状腺抗体与骨骼肌、骨密度之间的关系。方法选取在江苏大学附属昆山医院内分泌科住院的甲状腺素为正常范围内的T2DM患者389例。通过双能X射线骨密度仪测定受试者的骨骼肌质量和骨密度(bone mineral density,BMD),分析甲状腺素与骨骼肌质量、骨密度的相关性。根据甲状腺过氧化物酶抗体(thyroid peroxidase antibody,TPOab)滴度,将受试者分为TPOab阳性组和TPOab阴性组,比较两组间骨骼肌质量和BMD的差异。结果在男性T2DM患者中,FT3与体质量指数(body mass index,BMI)、总体脂肪质量和躯干脂肪质量呈正相关,FT4与四肢骨骼肌质量(appendicular skeletal muscle mass,ASM)呈负相关(P<0.05)。在女性T2DM患者中,FT4与BMI、总体骨骼肌质量、躯干骨骼肌质量和ASM呈负相关(P<0.05)。结论甲状腺素在正常范围内的T2DM患者,其FT4与ASM具有一定的相关性。Objective To investigate the relationship between thyroid hormones and thyroid antibodies and skeletal muscle and bone mineral density(BMD)in type 2 diabetic patients(T2DM)with normal range of thyroid hormones.Methods 389 T2DM patients with normal range of thyroid hormones in the Department of endocrinology of Kunshan Hospital Affiliated to Jiangsu University were collected.Skeletal muscle mass and BMD were measured by dual energy X-ray absorptiometry(DXA),and the correlation between thyroid hormone and skeletal muscle mass and BMD was analyzed.According to the thyroid peroxidase antibody(TPOab)titer,the subjects were divided into TPOab positive group and TPOab negative group.The differences of skeletal muscle mass and BMD between the two groups were compared.Results In male T2DM patients,FT3 was positively correlated with body mass index(BMI),total fat mass and trunk fat mass,and FT4 was negatively correlated with appendicular skeletal muscle mass(ASM),P<0.05.In women with T2DM,FT4 was negatively correlated with BMI,total skeletal muscle mass,trunk skeletal muscle mass and ASM(P<0.05).Conclusion FT4 is associated with ASM in euthyroid type 2 diabetic patients.
