摘 要:目的 对比分析某医院静脉用药调配中心HIS系统优化前后1年内的常见差错(指内差)发生率,旨在加强该医院静脉用药集中调配差错管理,为临床用药提供指导,确保临床用药安全。方法 查阅某医院静脉用药调配中心差错登记记录,从静脉用药集中调配的操作环节、常见差错类型两个角度,对静脉用药调配中心HIS系统优化前后1年内的差错发生率进行对比分析。结果 静脉用药调配中心HIS系统优化后1年内的差错发生率明显降低,在操作环节中,审核差错发生率下降最为明显,减少169例,下降至0.002%;在差错类型中,溶媒调剂错误发生率下降最为明显,减少164例,下降至0.005%。结论 智能化医院信息系统干预静脉用药集中调配差错发生的效果显著,有效保障用药安全。OBJEVTIVE To compare and analyze the incidence of common errors in one year before and after the optimization of HIS system in a hospital′s pharmacy intravenous admixture service(PIVAS).The aim is to strengthen the error management of the centralized intravenous drug dispensing in this hospital, provide guidance for clinical medication and effectively ensure the safety of patients’ medication.METHODS By reviewing the error registration records of the PIVAS in a hospital, the error rate in one year before and after the optimization of HIS system in the PIVAS were compared and analyzed from the aspects of operation links and the common error types.RESULTS After the optimization of HIS system in the PIVAS,the error rate decreased significantly within one year.In the operation link, the error rate of audit decreased most obviously, with 169 cases reduced to 0.002%;among the error types, the error rate of solvent dispensing decreased most obviously, with 164 cases reduced to 0.005%.CONCLUSION The intelligent hospital information system can effectively prevent the occurrence of errors in centralized dispensing of intravenous medication, and effectively ensure the safety of medication.
