摘 要:目的探讨SIRT1(Silent mating type information regulation 2 homolog-1,SIRT1)在畸形精子症患者精液中的表达水平及临床意义。方法选取2021年01月至2022年05月在河北医科大学第四医院生殖医学科就诊78例男性不育症患者为研究对象,收集精液标本,按WHO精液分析第5版标准进行精液分析,按精子正常形态分为两组,正常精子形态≥4为对照组(A组,n=38),正常精子形态<4为畸形精子组(B组,n=40),评估精液量、p H、精子浓度、前向精子百分比;酶联免疫吸附测定(enzyme linked immunosorbent assay,ELISA)检测精浆SIRT1含量;实时荧光定量PCR(Quantitative Real-time PCR,realtime PCR)检测精子SIRT1 mRNA表达。结果(1)A和B组精液量、p H、精子浓度和前向运动精子百分比两组间比较,差异无统计学意义(P>0.05),正常精子形态两组间比较差异有统计学意义(P<0.05)。(2)A组精浆丙二醛(Malonic dialdehyde,MDA)水平低于B组,超氧化物歧化酶(Superoxide Dismutase,SOD)水平高于B组,两组比较差异有统计学意义(P<0.05);A组精浆SIRT1水平高于B组,两组比较差异有统计学意义(P<0.05)。(3)精子形态正常与SIRT1活性呈正相关(r=117.0,P<0.001);精浆中MDA与SIRT1活性呈负相关(r=34.94,P<0.001),SOD与SIRT1活性呈正相关(r=100.7,P<0.001)。(4)实时荧光定量PCR结果显示,A组精子SIRT1 mRNA水平高于B组,两组比较差异有统计学意义(t=11.10,P<0.001)。结论畸形精子症患者精浆SOD水平降低、MDA水平升高、SIRT1活性降低,精子SIRT1 mRNA水平降低,SIRT1在畸形精子症中起作用可能与氧化应激有关。Objective To investigate the expression level of SIRT1 in semen of patients with teratozoospermia and its clinical significance.Methods A total of 78 male infertility patients,who were treated in the Department of Reproductive Medicine,the Fourth Hospital of Hebei Medical University from January 2021 to May 2022,were selected as the research subjects,and their semen samples were collected for semen analysis.Based on the result of sperm morphology,they were divided into two groups such as group A(sperm normal morphology≥4)and group B(sperm normal morphology<4,teratozoospermia group).The semen analysis including semen volume,p H,sperm concentration,percentage of progressive motility sperm was performed according to 5th edition WHO standard of semen analysis.The level of SIRT1 in seminal plasma was measured by enzyme Linked immunosorbent assay(ELISA).mRNA expression SIRT1 in sperm was detected by quantitative real-time PCR.Results(1)There were no significant differences in semen volume,sperm concentration,forward motility sperm and semen PH between groups A and B(P>0.05),but there was an obvious difference in sperm morphology between the two groups(P<0.05);(2)MDA level in group A was lower than that in group B,and SOD level was higher than that in group B,both differences were significant(P<0.05);The level of SIRT1 in group A was higher than that in group B,and the difference was statistically significant(P<0.05).(3)There was a positive correlation between sperm morphology and SIRT1 activity(r=117.0,P<0.001);MDA activity was negatively correlated with SIRT1 in seminal plasma(r=34.94,P<0.001),but SOD activity was positively correlated with SIRT1 in seminal plasma(r=100.7,P<0.001).(4)The mRNA expression level of SIRT1 in group A was higher than that in group B.There was a significant difference between the two groups(t=11.10,P<0.001).Conclusion The level of seminal plasma antioxidant enzyme decreased,the level of MDA increased,the activity of SIRT1 decreased,and the level of SIRT1 mRNA in sperm decreased in teratosp
