摘 要:目的研究小剂量骨水泥经皮椎体后凸成形术(PKP)治疗创伤性胸腰椎骨折的效果及对胸腰椎功能的影响。方法选取2016年10月-2020年10月天津市第四中心医院行PKP治疗的60例创伤性胸腰椎骨折患者,采用随机数字表法分为小剂量组(30例)与大剂量组(30例),分别给予小剂量骨水泥PKP治疗与大剂量骨水泥PKP治疗,比较两组VAS评分、伤椎高度、胸腰椎功能(脊柱ODI评分、腰椎JOA评分)、骨水泥渗漏情况、术后并发症发生情况及预后随访情况。结果两组术后VAS评分、ODI评分低于术前,伤椎前缘高度、伤椎中间高度、JOA评分高于术前(P<0.05),但组间比较,差异无统计学意义(P>0.05);小剂量组骨水泥渗漏率及并发症发生率均低于大剂量组(P<0.05);术后6个月,两组伤椎高度均无明显丢失,且无邻近椎体骨折及骨水泥移位现象。结论小剂量骨水泥PKP治疗创伤性胸腰椎骨折的效果确切,可保证良好的止痛及椎体恢复效果,同时降低患者的骨水泥渗漏及手术并发症风险,有利于脊椎功能的改善。Objective To study the effect of low-dose bone cement percutaneous kyphoplasty(PKP)in the treatment of traumatic thoracolumbar fractures and its effect on thoracolumbar function.Methods Sixty patients with traumatic thoracolumbar fractures who underwent PKP in Tianjin Fourth Central Hospital from October 2016 to October 2020 were selected.They were divided into low-dose group(30 cases)and high-dose group(30 cases)by random number table method.Low-dose bone cement PKP treatment and high-dose bone cement PKP treatment were given respectively.VAS score,vertebral height,thoracolumbar function(spinal ODI score,lumbar JOA score),bone cement leakage,postoperative complications and prognosis follow-up were compared between the two groups.Results The VAS score and ODI score after operation of the two groups were lower than those before operation,and the anterior height of the injured vertebra,the middle height of the injured vertebra and JOA score were higher than those before operation(P<0.05),but there was no significant difference between the two groups(P>0.05).The bone cement leakage rate and complication rate in the low-dose group were lower than those in the high-dose group(P<0.05).At 6 months after operation,there was no significant loss of vertebral height,adjacent vertebral fracture and bone cement displacement in both groups.Conclusion Low-dose bone cement PKP is effective in the treatment of traumatic thoracolumbar fractures,which can ensure good analgesic effect and vertebral recovery,reduce the risk of bone cement leakage and surgical complications,and is conducive to the improvement of spinal function.
