摘  要:目的:观察对比门静脉系统成像中低电压、低浓度对比剂的64排GE螺旋CT的应用效果。方法:于2019年10月~2021年10月在本院进行门静脉CT血管造影(CTA)检查的80例患者作为观察对象,运用数字随机表法设为四组,每组各20例。其中A组电压120kV、碘海醇浓度350mgI/mL;B组电压120kV、碘海醇浓度300mgI/mL;C组电压90~120kV、碘海醇浓度350mgI/mL;D组电压90~120kV、碘海醇浓度300mgI/mL。对比各组辐射剂量、图像质量、血肌酐与血尿素氮水平。结果:(1)辐射剂量。C组、D组的CT容积剂量指数、有效剂量、剂量长度乘积显著低于A组、B组(P<0.05);(2)图像质量。各组门静脉图像质量合格率无差异(P>0.05),A组CNR噪声比、SNR信噪比>B组>C组>D组(P>0.05);(3)肾功能。检查前血肌酐、血尿素氮明显低于检查后,且B组、D组血肌酐、血尿素氮明显低于A组、C组(P<0.05)。结论:临床应用低电压、低浓度对比剂的64排GE螺旋CT检查,能够有效满足临床诊断需要,不仅可以减少辐射剂量,而且还可以减轻对患者肾功能的不良影响。Objective:To observe the application effect of 64 rows of GE spiral CT with low voltage and low concentration of contrast agent in the portal vein system imaging.Methods:80 patients who underwent portal vein CT angiography(CTA)in our hospital from October 2019 to October 2021 were observed.They were divided into four groups with 20 patients in each group by using digital random table.The voltage of group A was 120k V,and the concentration of iohexol was 350mg I/m L;The voltage of group B was 120k V,and the concentration of iohexol was 300mg I/m L;The voltage of group C was of 90~120k V,and the concentration of iohexol was 350mg I/m L;The voltage of group D was 90~120k V,and the concentration of iohexol was 300mg I/m L.The radiation dose,image quality,blood creatinine and blood urea nitrogen levels were compared in each group.Result:(1)Radiation dose.The product of CT volume dose index and effective dose and dose length in groups C and D was significantly lower than that in groups A and B(P<0.05).(2)Image quality.There was no difference in portal vein image quality pass rate(P>0.05),CNR noise ratio,SNR signal to noise ratio>B>C>D(P>0.05).(3)Renal function.Blood creatinine and blood urea nitrogen were significantly lower than those after examination,and those in group B and C were significantly lower than those in group A and C(P<0.05).Conclusion:The clinical application of 64-row GE spiral CT examination with low voltage and low concentration contrast agent can effectively meet the clinical diagnosis needs,and can not only reduce the radiation dose,but also reduce the adverse effects on the renal function of patients.
