MRI联合CT对游离型腰椎间盘突出症的诊断价值

摘　　要：目的:分析游离型腰椎间盘突出症疾病特点,评价磁共振成像(Magnetic Resonance Imaging,MRI)联合计算机体层成像(Computed Tomography,CT)的诊断价值。方法:选择2018年8月~2020年6月本院诊治的80例游离型腰椎间盘突出症疾病患者,患者知情同意配合参与,经医院伦理委员会审核批准。根据影像学检查手段,80例游离型腰椎间盘突出症患者均行MRI检查、CT检查、MRI+CT检查。分析游离型腰椎间盘突出症患者的影像征像诊断情况,与手术病理比较检出结果。结果:征像诊断情况比较,MRI、CT检查钙化(40.00%,0)、积气(50.00%,0)、椎间盘变性(0,80.00%)、硬脊膜受压(35.00%,60.00%)、脊髓变性(0,15.00%)方面占比率差异显著(P<0.05);不同方法检出结果比较,MRI+CT检出率(98.75%)高于MRI(90.00%)、CT(77.50%)(P<0.05),且MRI检出率高于CT(P<0.05)。结论:MRI、CT均为游离型腰椎间盘突出症常用影像学诊断方式,对比单一诊断,MRI联合CT提升了疾病检出率,为游离型腰椎间盘突出症后续治疗工作提供参考。Objective:To analyze the characteristics of free lumbar disc herniation and evaluate the diagnostic value of magnetic resonance imaging(MRI)combined with computed tomography(CT).Methods:We selected 80 patients with free lumbar disc herniation who were diagnosed and treated in our hospital from August 2018 to June 2020,and the patients had informed consent to cooperate and participated,and were reviewed and approved by the hospital ethics committee.According to the imaging methods,all 80 patients with free lumbar disc herniation underwent MRI,CT,and MRI+CT.Analyze the image diagnosis of patients with free lumbar disc herniation,and compare the detection results with surgical pathology.Results:Comparison of signs and diagnosis,MRI,CT examination of calcification(40.00%vs0),pneumoconiosis(50.00%vs0),intervertebral disc degeneration(0vs80.00%),dural compression(35.00%vs60.00%),spinal cord degeneration(0vs15.00%)accounted for significant differences(P<0.05);The detection results of different methods were compared,the detection rate of MRI+CT(98.75%)was higher than that of MRI(90.00%),CT(77.50%)(P<0.05),and the detection rate of MRI was higher than that of CT(P<0.05).Conclusion:Both MRI and CT are commonly used imaging diagnostic methods for free lumbar disc herniation.Compared with a single diagnosis,MRI combined with CT improves the detection rate of the disease and provides a reference for the follow-up treatment of free lumbar disc herniation.
