基于CBCT结合微创拔牙在中低位阻生牙拔除中的临床应用

摘　　要：目的:分析应用锥形束CT(Cone Beam Computed Tomography,CBCT)结合微创拔牙法应用在中低位阻生牙拔除中的临床效果。方法:统计2017年1月~2022年9月在本院接受诊治的203例中低位阻生牙作为此次的研究对象,按拍片类型、与下颌神经管的接触关系、手术方法,分为两组,观察组与对照组各40例,对照组进行全景片加涡轮钻、锤凿法治疗,观察组实施CBCT引导下结合微创拔牙法,分析并发症、疼痛评分、牙槽窝不完全率、断根率、敲击增隙率。结果:观察组的并发症发生率和牙槽窝不完全率、敲击增隙率和断根率均低于对照组,且疼痛程度改善更优,组间的比较差异有统计学意义(P<0.05)。结论:在中低位阻生牙的拔除中实施CBCT引导下微创拔牙,术后并发症发生率低,对患者造成的反应小,可减轻疼痛,缩短拔牙手术时间,患者的认可度较高。Objective:To analyze the clinical effect of CBCT combined with minimally invasive extraction in the extraction of middle and low impacted teeth.Methods:From January 2017 to September 2022,203 patients with middle and low impacted teeth who were diagnosed and treated in our hospital were counted as the subjects of this study.Among them,40 patients were divided into two groups according to the type of radiograph,the contact relationship with the mandibular nerve canal,and the surgical method.The control group was treated with panoramic film,whirl wheel drill,and hammer chisel method.The observation group was treated with CBCT guided minimally invasive extraction method,and the complications,pain scores,incomplete alveolar fossa rate,root breakage rate,and tapping clearance rate.Results:The complication rate,the rate of incomplete alveolar fossa,the rate of increased space by tapping and the rate of root breakage in the observation group were lower than those in the control group,and the degree of pain was better improved.The difference between the groups was statistically significant(P<0.05).Conclusion:In the extraction of middle and low impacted teeth,the minimally invasive extraction under the guidance of CBCT has low postoperative complication rate,little reaction to patients,can reduce pain,shorten the extraction operation time,and is highly recognized by patients.
