摘 要:目的:探索经颅多普勒超声(Transcranial Doppler,TCD)在颅内血管狭窄中的诊断价值。方法:在本院2018年3月~2020年2月诊治的疑似颅内血管狭窄患者中选出133例为研究对象,均进行经颅彩色多普勒超声检查(TCD)和数字减影血管造影(Digital Subtraction Angiography,DSA)分析TCD对颅内血管狭窄的诊断价值。结果:本研究中DSA共检出128条狭窄血管,TCD共检出123条狭窄血管,TCD与DSA检查的诊断符合率为86.72%,其中大脑中动脉符合率为89.74%,颈内动脉的符合率为85.0%,椎动脉的符合率为75.76%,基底动脉的符合率为94.44%。TCD与DSA检查对狭窄程度的诊断符合率为92.79%,其中,闭塞与重度狭窄的符合率均为100.0%,中度狭窄的符合率为92.31%,轻度狭窄的符合率为81.82%。结论:颅内多普勒检查在颅内血管狭窄的诊断中应用价值高,能准确观察大脑动脉血管的血流动力学,测量血流速度、搏动指数等,能早期检出颅内动脉狭窄,且操作简单、无创、可重复性好,可作为颅内血管狭窄的筛查手段。Objective:To explore the diagnostic value of intracranial Doppler(TCD)in intracranial vascular stenosis.Methods:133 patients with suspected intracranial vascular stenosis were selected from March 2018 to February 2020,transcranial color Doppler ultrasound(TCD)and cerebral angiography(DSA),analysis of TCD diagnostic value for intracranial vascular stenosis.Result:A total of 128 narrow blood vessels DSA detected in this study,a total of 123 narrow blood vessels TCD detected,the coincidence rate of TCD and DSA was 86.72%,including 89.74%for middle cerebral artery,85.0%for internal carotid artery,75.76%for vertebral artery and 94.44%for basilar artery.The coincidence rate of TCD and DSA in the diagnosis of stenosis was 92.79%.The coincidence rate of occlusion and severe stenosis was 100.0%,the coincidence rate of moderate stenosis was 92.31%,and the coincidence rate of mild stenosis was 81.82%.Conclusion:Intracranial Doppler examination is valuable in the diagnosis of intracranial vascular stenosis,the hemodynamics of the cerebral arteries,measuring blood flow velocity,pulsatility index,early detection of intracranial artery stenosis,and simple,non-invasive,reproducible,it can be used as a screening method for intracranial vascular stenosis.
