超声引导下局部溶栓介入治疗下肢深静脉血栓的应用价值

摘　　要：目的:分析超声引导下局部溶栓介入治疗下肢深静脉血栓的应用价值。方法:选取2019年1月~2022年6月本院治疗的62例下肢深静脉血栓患者为研究对象,并依据患者的溶栓治疗为应用超声引导下局部溶栓介入治疗与常规溶栓治疗将其分入观察组(31例)和对照组(31例),对照组患者单纯采用常规溶栓治疗,观察组患者采用超声引导下局部溶栓介入治疗。比较两组患者溶栓前后患、健侧膝上15cm处和小腿最粗处周径改善情况以及肢体消肿率、溶栓率、总有效率。结果:观察组肢体消肿率、溶栓率均明显高于参考组(P均<0.05);两组溶栓前患、健侧膝上15cm处和小腿最粗处周径相比无明显差异(P均>0.05),溶栓后患、健侧膝上15cm处和小腿最粗处周径均显著改善(P均<0.05),且观察组患、健侧膝上15cm处和小腿最粗处周径改善情况明显优于对照组(P均<0.05);观察组痊愈25例,有效5例,无效1例,治疗总有效率为96.77%,对照组痊愈19例,有效6例,无效6例,治疗总有效率为80.65%,观察组下肢深静脉血栓治疗总有效率显著高于对照组(P均<0.05)。结论:超声引导下局部溶栓介入治疗下肢深静脉血栓能够获得更好的肢体消肿率、溶栓率及总体疗效,并促进患、健侧膝上15cm处和小腿最粗处周径改善,具有非常重要的应用价值。Objective:To analyze the application value of ultrasound-guided local thrombolytic interventional therapy for lower extremity deep vein thrombosis.Methods:Select during January 2019 to June 2022 in our treatment of 62 patients with lower extremity deep vein thrombosis as the research object,and on the basis of thrombolysis in patients with local thrombolysis therapy for the application of ultrasound guided interventional therapy with conventional thrombolysis treatment the points into research group(31 cases)and control group(31 cases),the control group only with conventional thrombolysis treatment,the study group received ultrasound-guided local thrombolytic interventional therapy.The circumference improvement of the affected and healthy side at 15cm above the knee and the thickest part of the calf before and after thrombolytic therapy were compared between the two groups,as well as the limb deswelling rate,thrombolytic rate and total effective rate.Results:The limb detumescence rate and thrombolysis rate of the study group were significantly higher than those of the reference group(all P<0.05).There was no significant difference in the circumference of 15cm above the knee and the thickest part of the calf between the two groups before thrombolytic therapy(all P>0.05),but the circumference of 15cm above the knee and the thickest part of the calf were significantly improved after thrombolytic therapy(all P<0.05).The improvement of circumference at 15cm above the knee and the thickest part of the calf in the study group was significantly better than that in the control group(all P<0.05).In the study group,25 cases were cured,5 cases were effective,1 case was ineffective,and the total effective rate was 96.77%.In the control group,19 cases were cured,6 cases were effective,6 cases were ineffective,and the total effective rate was 80.65%.The total effective rate in the study group was significantly higher than that in the control group(all P<0.05).Conclusion:Ultrasound-guided local thrombolytic interventional therap
