摘  要:目的探讨达芬奇机器人手术系统辅助下人工血管外支撑术治疗胡桃夹综合征(NCS)的临床疗效。方法收集2021年1—6月陆军军医大学第一附属医院收治的中国首例行达芬奇机器人手术系统辅助下人工血管外支撑术治疗的NCS患者的临床资料。观察患者的术前腹部计算机断层扫描血管成像(CTA)检查情况,术中、术后情况。采用门诊复诊的形式对患者进行随访,观察患者的术后一般情况及生存情况。结果患者成功完成达芬奇机器人手术系统辅助下人工血管外支撑术治疗,手术时间为325 min,术中出血量为100 ml,未出现血管损伤及术中、术后无并发症发生。术后随访10个月,患者生存良好,无腹痛、腰痛等症状,活动后无肉眼血尿。复查腹部CTA提示支撑环人工血管无明显移位,左肾静脉未见明显压迫,管腔恢复正常。结论达芬奇机器人手术系统辅助下人工血管外支撑术治疗NCS安全可行。Objective To explore the clinical efficacy of artificial vascular external support assisted by da Vinci robotic surgery system in the treatment of nutcracker syndrome(NCS).Method The clinical data of the first NCS patient in China who was admitted to the First Affiliated Hospital of Army Medical University from January to June 2021 and treated with artificial vascular external support assisted by da Vinci robotic surgery system.The preoperative abdominal computer tomography angiography(CTA)examination,intraoperative and postoperative conditions were observed.Follow up the patient in the form of outpatient follow-up to observe the general situation and survival of patients after surgery.Result The patient successfully completed the treatment of artificial vascular external support assisted by da Vinci robotic surgery system,the surgery time was 325 minutes,and the intraoperative blood loss was 100 ml,there was no vascular injury and no intraoperative or postoperative complications occurred.After 10 months of follow-up,the survival was satisfactory,abdominal pain,low back pain,and other symptoms were not reported,no gross hematuria was observed after exercise.A re-examination of abdominal CTA indicated that there was no obvious displacement of the artificial blood vessel of the support ring,no obvious compression of the left renal vein,and the lumen returned to normal.Conclusion It is safe and feasible to treat NCS with artificial vascular external support assisted by da Vinci robotic surgical system.
