桂枝加厚朴杏子汤加味对小儿病毒性肺炎中医证候评分及免疫功能的影响

摘　　要：目的 探讨桂枝加厚朴杏子汤加味对小儿病毒性肺炎中医证候评分及免疫功能的影响。方法 选取医院2020年1月—2021年12月78例小儿病毒性肺炎患儿,随机分为观察组与对照组,每组39例患儿。对照组给予常规西医治疗,观察组在对照组的基础上给予桂枝加厚朴杏子汤加味治疗,比较两组患儿临床疗效、中医证候积分、肺功能、免疫功能、炎性指标、不良反应发生情况。结果 观察组临床治疗有效率94.87%(37/39)高于对照组76.92%(30/39)(P<0.05);观察组治疗后咳嗽、喘急、鼻煽、发热、恶风、咽红临床症状积分低于对照组(P<0.05);观察组治疗后肺功能指标用力肺活量(FVC)、一秒用力呼气容积(FEV1)、第1秒用力呼气量占用力肺活量比值(FEV1/FVC)、峰值呼气流速(PEF)水平高于对照组(P<0.05);观察组治疗后外周血免疫球蛋白IgA、IgG、IgM水平以及T淋巴细胞CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平高于对照组,CD_(8)^(+)水平低于对照组(P<0.05);观察组治疗后炎性指标白细胞介素6(IL-6)、C反应蛋白(CRP)、肝素结合蛋白(HBP)、降钙素原(PCT)水平低于对照组;两组患儿不良反应发生率比较差异无统计学意义(P>0.05)。结论 在常规治疗的基础上,联合使用桂枝加厚朴杏子汤加味治疗小儿病毒性肺炎可有效降低患儿中医证候评分,提高肺功能水平,减轻炎性损伤,提升免疫功能。Objective To explore the effects of Guizhi and Houpo Xingzi Decoction(桂枝加厚朴杏子汤)on scores of TCM symptoms and immune function in children with viral pneumonia.Methods A total of 78 children with viral pneumonia in the hospital were enrolled between January 2020 and December 2021.They were randomly divided into observation group and control group,39 cases in each group.The control group was given routine western medicine,while the observation group was given Guizhi and Houpo Xingzi Decoction on basis of control group.The clinical curative effect,scores of TCM syndromes,lung function,immune function,inflammatory indexes and occurrence of adverse reactions were compared between the two groups.Results The response rate of clinical treatment in the observation group was higher than that in the control group[94.87%(37/39)vs 76.92%(30/39)](P<0.05).After treatment,scores of clinical symptoms(cough,wheezing,nasal congestion,fever,aversion to wind,throat redness)in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of lung function indexes[forced vital capacity(FVC),forced expiratory volume in one second(FEV1),FEV1/FVC,peak expiratory flow(PEF)]in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of IgA,IgG,IgM,CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) in the observation group were higher than those in the control group,while CD■level was lower than that in the control group(P<0.05).After treatment,the levels of interleukin-6(IL-6),C-reactive protein(CRP),procalcitonin(PCT)and heparin-binding protein(HBP)in the observation group were lower than those in the control group.The difference in incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion On the basis of routine treatment,Guizhi and Houpo Xingzi Decoction can effectively reduce scores of TCM symptoms,improve the lung function,alleviate the inflammatory damage and enhance the immune function in children with
