摘　　要：目的 明确自拟当归四逆汤加减对断指再植术后血管危象的影响。方法 收集余姚市中医医院手足外科2020年1月1日—2021年12月31日外伤性手指离断60例患者,离断时间在6 h以内,随机分为试验组和对照组各30例,两组患者均采用抗感染、抗凝、抗血管痉挛等常规治疗,试验组在常规治疗基础上予自拟当归四逆汤,分别于处理的12 h、48 h、72 h、7 d及15 d后进行视觉模拟(VAS)评分、患肢肿胀情况、功能恢复情况及循环内皮细胞(CEC)细胞数量评估及测量。结果 与对照组比较,试验组在处理的72 h、7 d及15 d的VAS评分均显著优于对照组,且差异具有统计学意义(P<0.05);试验组在7 d和15 d的肿胀情况优于对照组,且差异具有统计学意义(P<0.05)。试验组15 d后运动功能、日常活动、感觉恢复、血液循环状态、外观及恢复工作情况这些指标均显著优于对照组,且差异具有统计学意义(P<0.05)。与对照组比较,试验组在15 d后CEC细胞数量明显高于对照组,且差异具有统计学意义(P<0.05)。结论 自拟当归四逆汤加减具有改善断指再植术后血管危象的作用。Objective To clarify the effect of Self-made Danggui Sini Decoction(自拟当归四逆汤)on vascular crisis after replantation of severed finger.Methods Sixty patients with traumatic finger severance who were admitted to the Department of Hand and Foot Surgery at Yuyao Hospital of Traditional Chinese Medicine within 6 hours after severance were selected between January 1,2020 and December 31,2021.They were randomly divided into experimental group and control group with 30 cases in each group.All received conventional treatments such as anti-infection,anticoagulation and anti-vasospasm.On this basis,patients in the experimental group were additionally treated with Self-made Danggui Sini Decoction.12 h,48 h,72 h,7 d and 15 d later,visual analog scale(VAS)scores,swelling of the affected limb,functional recovery and circulating endothelial cell(CEC)count were evaluated and measured.Results Compared with the control group,the experimental group had significantly better VAS scores at 72 h,7 d and 15 d after treatment(P<0.05).Swelling in the experimental group was better than that in the control group on day 7 and day 15(P<0.05).15 d later,motor function,activities of daily living,sensory recovery,blood circulation,appearance and returning to work were significantly better in the experimental group than those in the control group(P<0.05).Compared with the control group,the experimental group had significantly higher CEC count on day 15(P<0.05).Conclusion Self-made Danggui Sini Decoction can improve vascular crisis after replantation of severed finger.
