针刺颈夹脊穴联合动态小面关节松动术治疗颈性眩晕的临床疗效评价

摘　　要：目的 探讨针刺颈夹脊穴联合动态小面关节松动术治疗颈性眩晕的临床疗效。方法 选取2019年7月—2020年2月在十堰市人民医院接受治疗的92例颈性眩晕患者作为研究对象,采用随机数表法分为观察组和对照组,每组46例。对照组患者给予动态小面关节松动术治疗,观察组患者给予针刺颈夹脊穴联合动态小面关节松动术治疗。分别于治疗前后,对两组患者症状与功能进行评分,检测患者左侧和右侧椎动脉及基底动脉最大血流速度,评估两组患者的临床疗效。结果 治疗后,观察组的总有效率显著高于对照组[93.48%(43/46) vs 78.26%(36/46),χ^(2)=4.389,P=0.036],两组患者量表各项评分和总分较治疗前均显著改善(P<0.05),且观察组较对照组显著改善(P<0.05);两组患者左侧椎动脉、右侧椎动脉及基底动脉血管血流速度较治疗前均明显改善(P<0.05),观察组较对照明显改善(P<0.05)。结论 针刺颈夹脊穴联合动态小面关节松动术可显著改善患者椎-基底动脉血流,改善患者颈性眩晕症状与功能,提高患者疗效。Objective This paper investigated the clinical efficacy of acupuncture cervical Jiaji acupoints combined with dynamic small-sided arthrodesis in the treatment of cervical vertigo.Methods Ninety-two patients with cervical vertigo treated in the hospital from July 2019 to February 2020 were selected as study subjects, and were divided into observation group and control group using the random number table method, with 46 cases in each group.The patients in the control group were treated with dynamic small-sided arthrodesis, and the patients in the observation group were treated with acupuncture of cervical Jiaji acupoints combined with dynamic small-sided arthrodesis.Before and after treatment, the symptoms and functions of patients in the two groups were scored, the maximum blood flow velocities of the left and right vertebral arteries and basilar arteries were detected, and the clinical efficacy of the two groups was evaluated.Results After treatment, the overall effective rate of the observation group was significantly higher than that of the control group[93.48%(43/46) vs.78.26%(36/46),χ^(2)=4.389,P=0.036],and the scores and total scores of the scale in both groups were significantly improved compared with those before treatment(P<0.05),and scores of the observation group were significantly improved compared with those of the control group(P<0.05).The blood flow velocity of the left vertebral artery, right vertebral artery and basilar artery in both groups was significantly higher than that before treatment.The blood flow velocities of the left vertebral artery, right vertebral artery and basilar artery in both groups improved significantly compared with those before treatment(P<0.05),and those of the observation group improved significantly compared with those of the control group(P<0.05).Conclusion Acupuncture at cervical Jiaji acupoints combined with dynamic small facet arthrodesis can significantly improve vertebrobasilar artery blood flow, the cervical vertigo symptoms and functions and the efficacy.
