自拟益肾壮骨汤联合椎体后凸成形术治疗骨质疏松脊柱压缩性骨折68例

摘　　要：目的 观察自拟益肾壮骨汤联合椎体后凸成形术(PKP)治疗骨质疏松脊柱压缩性骨折的临床效果及对中医证候积分、腰背疼程度、腰部功能及远期生活质量的影响。方法 选取2019年11月—2021年4月期间收治的68例骨质疏松脊柱压缩性骨折患者,根据随机数字表法将入组患者随机分为对照组和观察组,每组34例,两组均进行PKP治疗,观察组在以上基础给予自拟益肾壮骨汤,两组均持续治疗3个月。观察治疗前后两组中医证候积分、腰背疼程度、腰部功能及远期生活质量的变化,并对其疗效进行记录。结果 治疗后两组中医证候积分、疼痛视觉模拟(VAS)评分、腰部Oswestry功能障碍指数(ODI)及中国人骨质疏松症生存质量简明量表(COQOL)评分均得到显著改善(P<0.05),且观察组改善效果优于对照组(P<0.05);观察组临床总有效率显著高于对照组(P<0.05)。结论 自拟益肾壮骨汤联合PKP治疗骨质疏松脊柱压缩性骨折,疗效确切,不仅能显著改善患者症状及体征,减轻或消除腰背疼痛,还能提高其腰部功能,在促进生活质量的提升也具有重要作用。Objective To observe the clinical effect of Self-made Yishen Zhuanggu Decoction(自拟益肾壮骨汤)combined with percutaneous kyphoplasty(PKP)in the treatment of osteoporotic vertebral compression fractures(OVCF),and the influence on TCM symptom scores,low back pain,lumbar function and long-term quality of life.Methods Sixty-eight patients with OVCF admitted to the hospital between November 2019 and April 2021 were selected.According to the random number table method,the patients were divided into control group and observation group,34 cases in each.All underwent PKP.On this basis,patients in the observation group were treated with Self-made Yishen Zhuanggu Decoction.All patients were given 3 months of treatment.Changes in TCM symptom scores,low back pain,lumbar function and long-term quality of life before and after treatment were observed.The curative effect was recorded.Results After treatment,TCM symptom scores,visual analogue scale(VAS)scores,lumbar Oswestry disability index(ODI)scores and the Chinese Osteoporosis-targeted Quality-of-Life short questionnaire(COQOL)scores of the two groups were significantly improved(P<0.05),and the improvement was better in the observation group than that in the control group(P<0.05).The total clinical response rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Self-made Yishen Zhuanggu Decoction combined with PKP is effective in the treatment of OVCF,which can not only significantly improve the patients'symptoms and signs,alleviate or eliminate low back pain,but also improve lumbar function and quality of life.
