参松养心胶囊治疗冠心病心绞痛疗效观察

摘　　要：目的 探讨参松养心胶囊联合琥珀酸美托洛尔缓释片治疗冠心病心绞痛的疗效及对患者血脂指标、心率变异性的影响。方法 选取2019年10月—2021年10月该院收治的60例冠心病心绞痛患者作为研究对象,按随机数字法分为对照组和观察组各30例,对照组给予琥珀酸美托洛尔缓释片治疗,观察组在对照组基础上给予参松养心胶囊治疗,治疗4周。比较两组临床疗效、症状缓解和硝酸甘油用量、血脂指标[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]、心率变异性指标[窦性心搏RR间期的标准差(SDNN)、每隔5 min窦性心搏RR间期平均值标准差(SDANN)、相邻窦性心搏RR间期差值均方根(RMSSD)、相邻窦性心搏RR间期差值>50 ms占间期总数的百分比(PNN50)]、炎症因子指标[超敏C-反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]、血液动力学指标[全血高切黏度(HSV)、全血低切黏度(LSV)、血浆黏度(PSV)、红细胞聚集指数]及不良反应发生率。结果 观察组治疗有效率高于对照组[93.33%(28/30)vs76.67%(23/30)](P<0.05);治疗后观察组心痛发作持续时间短于对照组,心痛发作频率和硝酸甘油用量低于对照组(P<0.05)。治疗后两组TC、TG、LDL-C、hs-CRP、TNF-α、IL-6水平及HSV、LSV、PSV、红细胞聚集指数均降低,HDL-C、SDNN、SDANN、RMSSD、PNN50均升高(P<0.05);且观察组TC、TG、LDL-C、hs-CRP、TNF-α、IL-6水平及HSV、LSV、PSV、红细胞聚集指数均显著低于对照组,HDL-C、SDNN、SDANN、RMSSD、PNN50均显著高于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论 参松养心胶囊联合琥珀酸美托洛尔缓释片治疗冠心病心绞痛的疗效显著,可减轻临床症状,降低血脂和机体炎症反应,改善血流动力学和心率变异,且安全性好。Objective To investigate the curative effect of Shensong Yangxin Capsule(参松养心胶囊)combined with Metoprolol Succinate Sustained-release Tablets in the treatment of coronary heart disease with angina pectoris,and the influence on blood lipid indices and heart rate variability.Methods Sixty patients with coronary heart disease and angina pectoris admitted to the hospital from October 2019 to October 2021 were selected as the research subjects.The patients were divided into control group and observation group by random number table method,with 30 cases in each group.Patients in the control group were treated with Metoprolol Succinate Sustained-release Tablets,while those in the observation group were treated with Shensong Yangxin Capsule based on the treatment of the control group.Both groups underwent 4 weeks of treatment.The comparison was made between the two groups in terms of clinical effects,symptom relief,dosage of nitroglycerin,blood lipid indices[total cholesterol(TC),triglycerides(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)],heart rate variability indicators[standard deviation of RR intervals(SDNN),standard deviation of all 5-min mean normal RR intervals(SDANN),root mean square of successive differences(RMSSD),percentage of adjacent NN intervals differing by more than 50 milliseconds(PNN50)],inflammatory factors[high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],hemodynamic indicators[whole blood high shear viscosity(HSV),whole blood low shear viscosity(LSV),plasma viscosity(PSV),erythrocyte aggregation index]and the incidence of adverse reactions.Results The treatment response rate of the observation group was higher than that of the control group[93.33%(28/30)vs 76.67%(23/30)](P<0.05).After treatment,the duration of cardiac pain in the observation group was shorter than that in the control group.The attack frequency of cardiac pain and the dosage of nitroglycerin were lower than those in the control gro
