益肾化痰汤治疗慢性阻塞性肺疾病急性发作的疗效及对LPO、GSH-Px和CAT的影响

摘　　要：目的 探讨益肾化痰汤治疗慢性阻塞性肺疾病急性发作的疗效及对LPO、GSH-Px和CAT的影响。方法 将126例慢性阻塞性肺疾病急性加重期患者分为治疗组和对照组,每组63例,对照组依据慢性阻塞性肺疾病诊断治疗指南开展规范化的西药方案治疗,包括抗感染、解痉平喘,吸氧、雾化,化痰止咳等相关治疗。治疗组在对照组治疗的基础上,口服益肾化痰汤治疗。两组均治疗21 d。比较中医证候(咳嗽、咯痰、喘息、胸闷、心悸和乏力)积分变化,比较治疗总有效率,比较治疗前后肺功能指标变化和LPO、GSH-Px和CAT水平变化。结果 治疗组治疗总有效率高于对照组(P<0.05);治疗组治疗后中医证候积分较对照组低(P<0.05);治疗组治疗后FVC、FEV1、FEV1/FVC改善幅度优于对照组(P<0.05);两组治疗后LPO水平较治疗前降低,GSH-Px和CAT水平较治疗前升高,且治疗组治疗后LPO水平低于对照组,GSH-Px和CAT水平高于对照组(P<0.05)。结论 益肾化瘀汤和西药常规疗法相结合,可有效的改善慢性阻塞性肺疾病急性发作时的临床症状,改善肺功能,改善氧化应激反应,提高治疗效果,值得进一步研究探讨。Objective To investigate the therapeutic effect of Yishen Huatan Decoction(益肾化痰汤)on acute attack of chronic obstructive pulmonary disease(COPD)and its effects on Plasma lipid peroxide(LPO),glutathione peroxidase(GSH-PX)and catalase(CAT).Methods A total of 126 patients with acute exacerbation of COPD were divided into treatment group and control group,with 63 patients in the two groups,respectively.The control group on the basis of COPD guide carried out the standardization of diagnosis and treatment of western medicine treatment,including anti-infection,spasmolysis and asthma,oxygen,atomization,phlegm cough and relevant treatment.The treatment group on the basis of treatment in the control group,was given Yishen Huatang Decoction orally.Both groups were treated for 3 weeks.The scores of TCM syndromes and therapeutic effects,lung function indexes,levels of LPO,GSH-PX and CAT of the two groups were compared before and after treatment.Results The total effective rate of the treatment group was higher than that of the control group(P<0.05).After treatment,the two groups'TCM symptom score was reduced,and the score of the treatment group was lower than that of the control group(P<0.05).Lung function index of the two groups after treatment was significantly improved,and that of the treatment group was improved better than that of the control group(P<0.05).LPO level in the two groups after treatment was lower,the levels of GSH-PX and CAT were higher than those before treatment,and those of the treatment group were improved better than those of the control group(P<0.05).Conclusion Yishen Huatan Decoction combined with conventional western medicine therapy can effectively improve the clinical symptoms of acute attack of chronic obstructive pulmonary disease,the lung function,oxidative stress response and the therapeutic effect,which is worthy of further study and discussion.
