苯扎氯铵超声透入在混合痔术后水肿中的作用

摘　　要：目的探讨苯扎氯铵超声透入在混合痔术后水肿患者中的作用。方法选取2020年10月至2022年2月浙江大学医学院附属第二医院临平院区收治的96例混合痔术后水肿患者为研究对象,根据随机数字表法将其分为对照组和观察组,每组各48例。对照组患者采用温水坐浴,观察组患者采用苯扎氯铵超声透入治疗;比较两组患者术后第1、3、7、14天创面数字疼痛评分法(numerical rating scale,NRS)评分、水肿评分及临床疗效和不良反应。结果两组患者的术后NRS评分均随时间延长逐渐降低(P<0.05);术后第3、7、14天,观察组患者的NRS评分均显著低于对照组(P<0.05)。观察组患者的术后水肿评分随时间延长逐渐降低(P<0.05),对照组患者的水肿评分随时间延长先升高后降低(P<0.05);术后第3、7、14天,观察组患者的水肿评分均显著低于对照组(P<0.05)。观察组患者的总有效率显著高于对照组(93.75%vs.79.17%,P<0.05);两组患者的不良反应发生率比较差异无统计学意义(χ^(2)=0.261,P=0.610)。结论苯扎氯铵超声透入法能显著减轻混合痔术后患者的水肿、疼痛情况,疗效显著,且安全性良好。Objective To investigate the effect of ultrasound penetration of benzalkonium chloride in patients with postoperative edema of mixed hemorrhoids.Methods A total of 96 patients with postoperative edema of mixed hemorrhoids treated in Linping District,the Second Affiliated Hospital Zhejiang University School of Medicine from October 2020 to February 2022 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,with 48 cases in each group.The patients in control group used warm water sitz bath,while the patients in observation group were treated with benzalkonium chloride ultrasonic penetration.The wound numerical rating scale(NRS)score and edema score on the 1st,3rd,7th and 14th day after operation,clinical efficacy and adverse reactions were compared between the two groups.Results After operation,the NRS score of both groups decreased gradually with the extension of time(P<0.05),the NRS score of observation group was significantly lower than that of control group on the 3rd,7th and 14th day after operation(P<0.05).After operation,the edema score of observation group was gradually decreased with the extension of time(P<0.05),while the edema score of control group was firstly increased and then decreased with the extension of time(P<0.05).The edema score of observation group was significantly lower than that of control group on the 3rd,7th and 14th day after operation(P<0.05).The total effective rate of observation group was higher than that of control group(93.75%vs.79.17%,P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(χ^(2)=0.261,P=0.610).Conclusion Benzalkonium chloride ultrasonic penetration can significantly reduce the edema and pain of patients with mixed hemorrhoids after operation,with significant curative effect and good safety.
