零切迹自稳型颈椎间融合器在多节段颈椎病颈椎前路减压植骨融合术中的应用效果

摘　　要：目的评价零切迹自稳型颈椎间融合器(ROI-C)在多节段颈椎病颈椎前路减压植骨融合术(ACDF)中的应用效果。方法回顾性分析2014年12月至2021年9月采用ROI-C行ACDF治疗的36例多节段颈椎病患者的临床资料,分析患者的一般情况、临床指标、影像学指标及典型病例。结果术后1、3个月及末次随访时,患者的视觉模拟评分法(VAS)、日本骨科学会(JOA)评分均优于术前,且术后3个月及末次随访时优于术后1个月,差异具有统计学意义(P<0.05);末次随访以JOA评分改善率评价疗效,优15例,良18例,可3例,优良率91.67%。术后3个月及末次随访时,患者的颈椎曲度及椎间隙高度均优于术前,差异具有统计学意义(P<0.05);末次随访发现颈椎曲度及椎间隙高度数值稍有下降,但与术后3个月比较,差异无统计学意义(P>0.05);末次随访时融合节段107个,融合率95.54%;1例术后3个月随访Cage沉降,术后6个月随访Cage位置同前,椎间融合;术后随访2年,1例出现影像学邻近节段退变征象,无临床症状。结论应用ROI-C进行多节段ACDF能有效地恢复颈椎病患者的颈椎生理曲度及椎间隙高度,为颈椎提供即刻稳定性,可获得良好的临床效果和满意的融合率。Objective To evaluate the application effect of zero notch self stabilizing cervical fusion cage(ROI-C)in anterior cervical decompression and fusion(ACDF)for multilevel cervical spondylosis.Methods The clinical data of 36patients with multilevel cervical spondylosis treated by ACDF with ROI-C from December 2014 to September 2021 were retrospectively analyzed,and the general condition,clinical indicators,imaging indicators and typical case charts of the patients were analyzed.Results At 1,3 months after operation and at the last follow-up,the Visual Analogue Scale(VAS)and Japanese Orthopaedic Association(JOA)scores of patients were better than those before operation,and those at 3months after operation and at the last follow-up were better than 1 month after operation,and the differences were statistically significant(P<0.05);at the last follow-up,the improvement rate of JOA score was used to evaluate the curative effect,15 cases was excellent,18 cases was good,3 cases were fair,and the excellent and good rate was 91.67%.At 3months after operation and at the last follow-up,the cervical curvature and intervertebral space height of the patients were better than those before operation,and the differences were statistically significant(P<0.05);at the last follow-up,it was found that the cervical curvature and intervertebral space height decreased slightly,but there were no significant differences compared with those at 3 months after operation(P>0.05);there were 107 fusion segments at the last follow-up,and the fusion rate was 95.54%;one Cage subsidence was found at 3 months after operation,and it remained stable after 6months of follow-up and reached fusion;after 2 years of follow-up,1 patient showed imaging signs of adjacent segmental degeneration without clinical symptoms.Conclusion The application of ROI-C in multilevel ACDF can effectively restore the cervical physiological curvature and intervertebral space height of patients with cervical spondylosis,provide immediate stability for cervical spine,and obtain goo
