摘 要:目的探讨基于气道分级管理策略的胸部物理疗法用于机械通气患者气道管理中对通气状态及肺部感染情况的影响。方法选择2019年2月至2021年3月我院收治的100例机械通气患者,根据干预方案将其分为对照组和观察组,各50例。对照组接受常规胸部物理治疗,观察组给予基于气道分级管理策略的胸部物理疗法。比较两组的干预效果。结果干预后,观察组的每日排痰量多于对照组,临床肺部感染评分(CPIS)评分低于对照组(P<0.05)。干预后,观察组的动脉血氧分压(PaO_(2))、血氧饱和度(SaO_(2))高于对照组,动脉血二氧化碳分压(PaCO_(2))低于对照组(P<0.05)。观察组的机械通气时间、重症监护病房(ICU)住院时间短于对照组,肺部感染发生率低于对照组(P<0.05)。结论基于气道分级管理策略的胸部物理疗法用于机械通气患者气道管理中,可有效改善通气状态及肺部感染情况,缩短机械通气时间和ICU住院时间。Objective To explore the effects of chest physiotherapy based on airway hierarchical management strategy on ventilation status and pulmonary infection in airway management of patients with mechanical ventilation.Methods One hundred patients with mechanical ventilation admitted in our hospital from February 2019 to March 2021 were selected and divided into control group and observation group according to the intervention plan,with 50 cases in each group.The control group received routine chest physiotherapy,and the observation group received chest physiotherapy based on airway hierarchical management strategy.The intervention effects of the two groups were compared.Results After intervention,the daily sputum output in the observation group was more than that in the control group,and the Clinical Pulmonary Infection Score(CPIS)was lower than that in the control group(P<0.05).After intervention,the arterial partial pressure of oxygen(PaO_(2))and oxygen saturation(SaO_(2))in the observation group were higher than those in the control group,and the arterial carbon dioxide partial pressure(PaCO_(2))was lower than that in the control group(P<0.05).The mechanical ventilation time and intensive care unit(ICU)hospitalization time in the observation group were shorter than those in the control group,and the incidence of pulmonary infection was lower than that in the control group(P<0.05).Conclusion Chest physiotherapy based on airway hierarchical management strategy applied to airway management of patients with mechanical ventilation can effectively improve the ventilation status and pulmonary infection,shorten the mechanical ventilation time and ICU hospitalization time.
