摘　　要：目的:探究甘露醇、呋塞米联合白蛋白治疗脑外伤后脑水肿的临床价值。方法:选取我院于2019年5月—2022年5月收治的90例外伤后脑水肿患者为研究对象,采取随机抽签分组方式分为观察组(n=45)和对照组(n=45)。对照组给予呋塞米联合甘露醇治疗,观察组在对照组治疗基础上给予白蛋白,比较两组临床疗效、不同时间尿量、不良反应以及水肿体积与24 h小便量。结果:观察组治疗总有效率高于对照组(P<0.05);观察组用药2、4、8、12及24 h后尿量多于对照组(P<0.05);观察组电解质紊乱、脑水肿反跳、肾功能异常发生率低于对照组(P<0.05);观察组水肿体积小于对照组,24 h小便量优于对照组(P<0.05)。结论:甘露醇、呋塞米、白蛋白三药联合治疗脑外伤后脑水肿的效果良好,可减轻患者因疾病产生的相关症状,不良反应少,安全性高。Objective:To explore the clinical value of mannitol,Furosemide combined with albumin in the treatment of cerebral edema after traumatic brain injury.Methods:Ninety patients with posttraumatic brain edema admitted to our hospital from May 2019 to May 2022 were selected as the research objects and divided into observation group(n=45) and control group(n=45) by random drawing.The control group was treated with Furosemide combined with mannitol,and the observation group was treated with albumin on the basis of the treatment of the control group.The clinical efficacy,urine volume at different times,adverse reactions,edema volume and 24 h urine volume were compared between the two groups.Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).The urine volume of the observation group was higher than that of the control group after 2,4,8,12 and 24 h of medication(P<0.05).The incidences of electrolyte disturbance,cerebral edema rebound and abnormal renal function in the observation group were lower than those in the control group(P<0.05).The edema volume of the observation group was smaller than that of the control group,and the 24 h urine volume of the observation group was better than that of the control group(P<0.05).Conclusion:The combination of mannitol,furosemide and albumin has a good effect in the treatment of cerebral edema after traumatic brain injury,which can reduce the related symptoms caused by the disease,with less adverse reactions and high safety.
