低管电压和低碘浓度溶剂联合自适应统计迭代重建V技术在主动脉CT造影扫描中的可行性研究

摘　　要：目的:探析低管电压和低碘浓度溶剂联合自适应统计迭代重建V(ASIR-V)技术在主动脉CT血管造影(CTA)扫描中的可行性。方法:选择医院收治的似诊为主动脉疾病行胸腹主动脉CTA检查的104例患者,所有患者均行主动脉CTA扫描检查,根据CTA扫描参数的不同分为对照组(48例)和观察组(56例),对照组采用管电压120 kVp、对比剂350 mg I/ml,观察组采用管电压80 kVp、对比剂320 mg I/ml。测量两组CT图像中的升主动脉、降主动脉、腹主动脉、髂总动脉和股动脉的CT值以及噪声标准偏差(SD)值,并计算管腔内的信噪比(SNR),记录CT容积剂量指数(CTDI_(vol))、剂量长度乘积(DLP),计算有效辐射剂量(ED)及碘剂量。按照欧洲放射学会指南推荐的CT图像质量相关评分标准5分法主观评价图像质量。结果:观察组升主动脉、降主动脉、腹主动脉、髂总动脉及股动脉节段中的CT值水平明显高于对照组,差异有统计学意义(t=6.508,t=7.692,t=12.460,t=7.964,t=7.736;P<0.05),升主动脉、降主动脉及股动脉节段中的SD值水平明显高于对照组,差异有统计学意义(t=3.947,t=2.724,t=1.487,t=2.891;P<0.05),升主动脉、降主动脉、腹主动脉及髂总动脉节段中的SNR值水平明显高于对照组,差异有统计学意义(t=3.182,t=3.257,t=3.732,t=3.927;P<0.05),CTDI_(vol)、DLP、ED及碘剂用量水平明显低于对照组,差异有统计学意义(t=16.836,t=18.073,t=13.654,t=21.922;P<0.05)。两组患者图像质量主观指标评分差异无统计学意义。结论:低管电压和低碘浓度溶剂联合ASIR-V技术行主动脉CTA扫描检查可有效降低患者辐射剂量及碘摄入量,并能获得优质的图像质量,为临床提供精准诊断依据。Objective:To analyze the feasibility of low tube voltage and low iodine concentration solvent combined with adaptive statistical iterative reconstruction-v(ASIR-V)in the scan of computed tomography angiography(CTA)of aorta.Methods:A total of 104 patients with suspected aortic disease who admitted to hospital and underwent CTA examination on aortas of breast and abdomen were selected.All of them underwent CTA scan examination,and they were divided into control group(48 patients)and observation group(56 patients)according to the different parameters of CTA scan.The control group adopted 120 kVp tube voltage and 350 mgI/mL contrast agent,and the observation group adopted 80 kVp tube voltage and 320 mgI/mL contrast agent.The CT values and noise standard deviation(SD)values of the ascending aorta,descending aorta,abdominal aorta,common iliac artery and femoral artery in the CT images were measured,and the signal-to-noise ratio(SNR)in the lumen was calculated.The CT volumetric dose index(CTDI_(vol))and dose length product(DLP)were recorded,and the effective radiation dose(ED)and iodine dose were calculated.The image quality was evaluated subjectively according to the 5-point scale of relevant scoring standards of CT image quality recommended by the European Society of Radiology guidelines.Results:There were no significant differences in age,gender,body mass index(BMI)and scan length between the two groups(P>0.05).The levels of CT values in the segments of ascending aorta,descending aorta,abdominal aorta,common iliac artery and femoral artery of observation group were significantly higher than those of control group(t=6.508,t=7.692,t=12.460,t=7.964,t=7.736,P<0.05),and the levels of SD values in the segments of ascending aorta,descending aorta and femoral artery of observation group were significantly higher than those of control group(t=3.947,t=2.724,t=1.487,t=2.891,P<0.05),respectively.The levels of SNR values in the segments of ascending aorta,descending aorta,abdominal aorta and common iliac artery of observation gro
