摘　　要：目的:探讨经阴道三维能量多普勒超声血流显像监测排卵障碍性不孕患者子宫内膜容受性及预测妊娠的临床价值。方法:选取医院收治的68例排卵障碍性不孕患者均给予促排卵治疗,根据促排卵治疗后妊娠情况将其分为妊娠组(28例)和未妊娠组(40例)。在注射人绒毛膜促性腺激素(HCG)当日,对两组采用三维阴道超声及三维能量多普勒超声检查,获取子宫内膜容积、内膜血管指数(VI)、血流指数(FI)和血管血流指数(VFI),并检测雌二醇及孕酮水平。结果:妊娠组与未妊娠组子宫内膜血流分型比较差异无统计学意义。妊娠组内膜容积及内膜VFI均高于未妊娠组,差异有统计学意义(t=3.912,t=2.149;P<0.05)。子宫内膜容积预测两组患者妊娠的子宫内膜容积受试者工作特征曲线下面积(AUC)为0.834,灵敏度为75.00%,内膜VFI的AUC为0.715,灵敏度为71.40%。结论:经阴道三维能量多普勒超声血流显像可较好地监测排卵障碍性不孕患者子宫内膜容积及内膜VFI,以及评估子宫内膜容受性。Objective:To investigate the clinical value of transvaginal three-dimensional(3D)power Doppler ultrasound flow imaging in monitoring endometrial receptivity and predicting pregnancy of patients with infertility caused by ovulation failure.Methods:68 patients with infertility caused by ovulation failure were collected.All of them were divided into the pregnant group(28 cases)and the non-pregnant group(40 cases)according to the pregnancy condition after treatment.All patients underwent therapy with ovulation induction.3D transvaginal ultrasound and 3D power Doppler ultrasound examination were adopted to obtain the endometrial volume,endometrial vascular index(VI),flow index(FI),vascularization-flow index(VFI),etc.on that day of injecting human chorionic gonadotropin(HCG).The levels of estradiol and progesterone were detected at the same time.Results:There was no statistically significant difference in endometrial flow classification between the pregnant group and the non-pregnant group(P>0.05).The endometrial volume and endometrial VFI in pregnant group were significantly higher than those in non-pregnant group,and the differences of them between two groups were statistically significant(t=3.912,t=2.149,P<0.05),respectively.The area under curve(AUC)of receiver operating characteristic(ROC)curve and sensitivity of endometrial volume were 0.834 and 75.00%,respectively.The AUC and sensitivity of endometrial VFI were 0.715 and 71.40%,respectively.Conclusion:Transvaginal 3D power Doppler ultrasound flow imaging can better monitor endometrial volume and endometrial VFI of patients with infertility caused by ovulation failure,and evaluate the endometrial receptivity of them.
